The carotid artery is opened and the plaque is removed. After removing the plaque the shunt is removed and the carotid artery is stitched and the blood flow is restored. From that time we started carrying out this procedure mainly under local anaesthetic with minimal monitoring. Initially we used cervical plexus blocks but now use simple local anaesthetic infiltration.
