List and identify on a chest radiograph and CT four patterns (nodular, reticular, reticulonodular, and linear) of interstitial lung disease. Separate pulmonary from pleural or extrapleural processes. Correlate radiographic and chest CT findings. Perform and interpret more complex post-processing (3D) images. Behavior and Attitude Objectives:

6 21 A solitary pulmonary nodule in the R mid zone. Adjacent to it is a large mass in the R upper mediastinum. The mediastinal mass is due to enlarged paratracheal lymph nodes. The solitary pulmonary nodule could be due to a primary lung neoplasm with associated mediastinal adenopathy.

A chest CT was reviewed which showed a suspicious pulmonary nodule in the upper lobe of his right lung. He had a PET/CT scan which revealed uptake by the right upper lobe pulmonary nodule as well as lymph nodes in the mediastinum and right hilum.
INTERVENTIONAL RADIOLOGY OF THE CHEST
X-ray computed tomography provides an effective means of detecting and monitoring pulmonary nodules, and can lead to increased survival (1) and reduced mortality (2) in individuals at high risk for lung cancer. Size quantification on serial imaging is helpful in evaluating whether a pulmonary nodule is benign or malignant.

In a retrospective study on identification of characteristics of solitary pulmonary nodule, the sensitivities of CT, PET and PET/CT were 93%, 69% and 97% while specificities were 31%, 85% and 85%. PET/CT has been shown to be invaluable for detecting distant metastasis, particularly in patients already known to have a primary tumor.

w/u of new pulmonary nodule: IS IT NEW? COMPARE WITH OLD FILMS!-- must exclude malignancy-- stability over 2 years is an extremely high indicator of benignancy-- dense and/or central calcification -- granuloma, hamartoma-- if new: 1) CT to assess for calcification, other nodules, adenopathy. 2) biopsy: bronchoscopy +/- lavage if central

A solitary pulmonary nodule is defined as a single discrete pulmonary opacity that is surrounded by normal lung tissue and is not associated with adenopathy or atelectasis. Radiographically, a nodule is defined as a lesion smaller than 3 cm. Anything larger than 3 cm is termed a mass. Provide possible diagnosis(es): Neoplasm: Benign or malignant
