What should be included in a diagnosis?
Integrate and interpret from a broad perspective all relevant history and assessment information. Include signs and symptoms or problems that justify the diagnosis as well as strengths or barriers that could impact treatment. Include important facts or events in the person's life, as applicable.
How do you write a differential diagnosis?
For each diagnosis discuss physiologic disease basis relevant to the patient and elements from the patient’s history and physical that either support or refute the diagnosis. For each item on your differential, explain what makes it likely AND what makes it less likely.
