Right arm and leg are able to move freely. Right hand –strong grip, right shoulder shrug, hip strength, hip extension, knee extension +5 in strength. Sensation on right is +5. Gait is uneven, left leg swings out while walking. Reflexes on right at bicep and patella +2. Sensation is WNL.

The patient demonstrates good external rotation and abduction to around 90 degrees with pain. Given the patient’s mechanism of injury and report of a ‘snapping’ sensation, the patient is asked to flex his bicep muscles bilaterally. The right bicep muscle demonstrates a prominent muscle bulge compared to the contralateral side.

The brachialis’ pain pattern is also shown on page 21 or on page 661 of the text. The bicep has been retracted to show four of the brachialis’ documented trigger points. The brachialis has some interesting pain patterns. The secondary pain pattern is very similar to what you just drew for the biceps.

The tenderness may be localized best with the arm in 100 of external rotation. Flexion of the elbow against resistance aggravates the patient's pain. Passive abduction of the arm in an arc maneuver may elicit pain that is typical of impingement syndrome; however, this finding may be negative in cases of isolated bicipital tendinitis.
