The yolk sac has nutritive, endocrine, metabolic, immunologic, secretory, excretory and haemopoetic functions. The purpose of this study are to study the biometry of the yolk sac and its relation with menstrual age, crown rump length and mean gestational sac diameter in early pregnancy and to assess the predictive value of abnormal yolk sac size for abnormal pregnancy outcome.

Yolk sac was identified from 34 days of gestation by transvaginal sonography and was identified from 42 days of gestation by transabdominal sonography. 117 first trimester singleton pregnancies were studied by Stampone C, Nicotra M, Muttinelli C, Cosmi EV 2 to evaluate yolk sac size and abnormalities using transvaginal ultrasound.

At 5-6 weeks EGA, a yolk sac becomes visible; it then shrinks and disappears by about 12 weeks. Around 5-6 weeks, an embryo (“fetal pole”) also appears by transvaginal ultrasound at the border of the yolk sac; the normal embryo will grow rapidly, around 1mm per day. Cardiac activity should be seen by the time the embryo is 5mm in length

No yolk sac or embryo is evident, too early to confirm live intrauterine pregnancy. There are no specific ultrasound features to suggest an ectopic pregnancy, but this cannot be entirely excluded. If appropriate, βhCG correlation and a follow-up scan in 14 days could be considered, if there is low clinical likelihood of ectopic pregnancy.
