Open chart to physicians section and find Resident Weight Record. Record new resident weight information from Weighing Worksheet onto Resident Weight Record in residents chart. In Notes section list new information that may affect residents weight, e.g. edema, casts, etc. Plot residents weight on the graph on the back side of Resident Weight Record. Note: Residents on weekly weights will have only bi-monthly weights plotted on graph. Significant Weight Changes: Weight Change Spreadsheet Turn on computer and open Weight Change Spreadsheet. At the beginning of the week, you will have to add a column for the weights that will be measured that week. To do this, hold down the Ctrl key and then press a. Type in date of Monday of current week in the new column that appears. If column for current week has already been created and dated, skip this step. Fill in new weight under current weeks column As you enter a current weight for each resident, the computer will automatically calculate the amount of weight loss or gain the resident has experienced within the last 7, 14, 30, 90, and 180 days. Look for significant weight changes by scanning the various Weight Change columns. If there are significant changes: Indicate the changes on Resident Weight Record in residents chart. (See below for definitions of a significant weight change). Fill out Weight, Food and Hydration Notification form to give to RD/ DTR. If there are not significant changes, you are finished with this resident. Repeat procedure (starting top of this page) with all residents weighed that day. Save newly entered data in Weight Change Spreadsheet. Click on the word File at the top of the screen until menu appears. Move the pointer down the menu to the Save As and click on it. Name the file Weight Change Spreadsheet date using the date of the current week. Click on the Save button. For the remainder of the week the file should be saved with the exact same date. Definition of Significant Weight Change A significant weight change is defined as: A resident who has a 3 lbs weight loss in 7 days. A resident who has a 3 lbs weight gain in 7 days. For a resident with a BMI greater than 19, a significant weight loss is: 3% weight loss in 14 days, 5% weight loss in 30 days, 7.5% weight loss in 90 days, 10% weight loss in 180 days.
