Evacuation begins when medical personnel receive the injured or ill Soldier and continues as far rearward as the patient's medical condition warrants or the military situation requires. Procedures for requesting medical evacuation support must be institutionalized down to the unit level. The same format used to request aeromedical evacuation is also used for requesting ground evacuation. NOTE: Tell the students: The medical evacuation request is used for requesting evacuation support for both air and ground ambulances. Before transmitting the request you must collect information for each of the nine lines and prepare it in the correct format. A copy of the MEDEVAC request format is in your handout. a. Location of Pickup Site (Line 1). Obtain the grid coordinates of the pickup site (see task 071-329-1002, Determine the Grid Coordinates of a Point on a Military Map) from the unit leader or the grid map of the operational area. This information is required so that the evacuation vehicle crew knows where to pick up the casualty and the unit personnel coordinating the evacuation mission can plan the route for the evacuation vehicle (if casualties must be picked up from more than one location). b. Radio Frequency, Call Sign, and Suffix (Line 2). Obtain the radio frequency, call sign, and suffix of the signal operation instructions (SOI) from the radio telephone operator (RTO). This information is required so that the evacuation vehicle crew can contact the requesting unit while en route to obtain additional information (for example, a change in situation, directions, or other information). c. Number of Patients by Precedence (Line 3). Obtain this information, a part of the evaluation of the patients, from the medic or the senior person present. This information is required by the unit controlling the evacuation vehicles in order to assist in prioritizing missions when more than one is received. The precedence provides the supporting medical unit and controlling headquarters with information that they can use to determine priorities for committing their evacuation assets. For this reason, correct assignment of a precedence cannot be overemphasized; overclassification remains a continuing problem. Overclassification wastes time and assets and could ultimately be the cause of a Soldier not receiving the care he needs. Patients will be picked up as soon as possible, consistent with available resources and pending missions. d. Special Equipment Required (Line 4). Obtain this information, a part of the evaluation of the patients and the situation, from the medic or the senior person present. Equipment to be included in a MEDEVAC request is limited to a hoist, extraction equipment, and ventilator. This information is required so that the equipment can be placed on board the evacuation vehicle prior to the start of the mission. e. Number of Patients by Type (Line 5). Obtain information on the number of litter and ambulatory patients from the medic or the senior person present. This information is required to determine the appropriate number of evacuation vehicles to be dispatched to the pickup site. The information is also needed to configure the vehicles to carry the patients requiring evacuation. f. Security of Pickup Site (Wartime) (Line 6). Obtain this information, an evaluation of the situation, from the unit leader. This information is required to assist the evacuation crew in assessing the situation and determining if assistance is required in accomplishing the mission. More definitive guidance can be furnished the evacuation vehicle while it is en route (for example, the specific location of the enemy would assist the crew in planning the approach). NOTE: Tell the students: The format also shows information to be transmitted during peacetime for lines 6 and 9. We are training to wartime standards and you will NOT be evaluated on peacetime requirements. g. Method of Marking Pickup Site (Line 7). This information, based on the situation and the availability of materials, is provided by the medic or senior person present. The information is required to assist the evacuation aircraft crew in identifying the specific location of the pickup site. The color of the panels, smoke, or other markings should not be transmitted until the evacuation vehicle contacts the unit just prior to arrival. For security reasons, the crew should identify the color of the markings and the unit should verify the color. h. Patient Nationality and Status (Line 8). The number of patients in each category need not be transmitted. Obtain this information from the medic or senior person present. The information is required in planning for destination facilities and the need for guards. The unit requesting evacuation support should ensure that there is an English-speaking representative at the pickup site. I. CBRN Contamination (Wartime) (Line 9). Obtain chemical, biological, radiological, and nuclear (CBRN) contamination information from the medic or senior person present. Include this line only when applicable. This information is required to assist in planning for the mission. It is used to determine which evacuation vehicle will accomplish the mission and when it will be accomplished. NOTE: Tell the students: Information concerning the vehicle to be used and the time of arrival can be obtained from the evacuation unit. NOTE:Conduct a check on learning and summarize the learning activity. 2. Learning Step / Activity 2. Record the gathered MEDEVAC information using authorized brevity codes as appropriate. The authorized codes are provided in the MEDEVAC request format; they are also provided in the SOI. Use of locally devised brevity codes is not authorized. The brevity codes must be used in secure and nonsecure communications to keep the transmission as short as possible. If the unit preparing the request does not have access to secure communications, the medical evacuation request must be prepared in encrypted form. Encrypting is required for all information on the request with the exception of— lš š The medical evacuation line number identifier. This information is always transmitted in clear text. lš š The call sign and suffix (Line 2) which can be transmitted in clear text. NOTE: Show VGT 0101-3, Location of Pickup Site. a. Location of Pickup Site (Line 1). Record the grid zone letters and coordinates. NOTE: Tell the students: When using DRYAD Numeral Cipher, the same SET line is used to encrypt both the grid zone letters and the coordinates (Line 1 of the request). To avoid misunderstanding, a statement should be made that the grid zone letters are included in the message. This must be accomplished unless the tactical standing operating procedures (TSOP) specifies that the DRYAD Numeral Cipher is to be used at all times. NOTE: Show VGT 0101-4, Radio Frequency, Call Sign, and Suffix. b. Radio Frequency, Call Sign, and Suffix (Line 2). Record the frequency of the radio at the pickup site, not a relay frequency. The call sign (and suffix if used) of the person to be contacted at the pickup site may be transmitted in the clear. NOTE: Show VGT 0101-5, Number of Patients by Precedence. c. Number of Patients by Precedence (Line 3). Report only applicable information using the brevity code(s) and amount(s). NOTE: Tell the students: The brevity code precedes the description (A - Urgent) in the line number block under the information column. Therefore if you have one urgent casualty, you would record A1. If two or more categories must be reported in the same request, insert the word “BREAK” between each category. The MEDEVAC request should be made by the most direct communication means to the unit that controls evacuation assets. The communication means and channels used depend on the situation (organization, communication means available, location on the battlefield, and distance between units). NOTE: The primary and alternate channels to be used for requesting MEDEVAC are specified in the unit evacuation plan. b. Contact the unit that controls the evacuation assets. Under all wartime conditions, these requests are transmitted by secure means only. Therefore, the use of nonsecure communications dictates that the request be transmitted in encrypted form. Regardless of the type (secure or nonsecure) communication equipment used in transmission, it is necessary to-- (1) Make proper contact with the intended receiver. Information about the intended receiver is obtained from the senior leader present who in turn obtains the information from the unit SOP, comment in the SOI, briefing for the mission, and/or a paragraph in the unit operation order (OPORD). NOTE: Remind the students about previous training on task 113-571-1022, ”Perform Voice Communications“. (2) Use the effective call sign and frequency designators from the SOI. (3) State the purpose of the radio message by providing the opening statement: "I have a MEDEVAC request, over." Wait 1 to 3 seconds for acknowledgment; if there is no answer or if contact is interrupted, repeat the statement, "I have a MEDEVAC request, over." c. Transmit the MEDEVAC information in the proper sequence. (1) State all item line numbers in clear text. The call sign and suffix (if used) in line 2 may also be transmitted in the clear. (2) Transmit other required information using the appropriate brevity codes and format. (3) Pronounce letters and numbers in accordance with appropriate radio telephone procedures. (4) Transmit the required information as quickly as possible. NOTE: Line numbers 1 through 5 of the request must always be transmitted during initial contact with the evacuation unit. Lines 6 through 9 may be transmitted while the aircraft or vehicle is en route. (5) End the transmission by stating, "Over." (6) Listen for an acknowledgment. (7) After transmission and acknowledgment are accomplished, the transmitting operator must monitor the frequency for additional instructions or contact from the evacuation unit. NOTE: Refer the students to the EXAMPLE MEDEVAC REQUEST located in their handout 081T0101. Remind the students that the example starts after initial contact is made with the unit controlling the evacuation; it is designed for use with a secure communication system. SECTION V. tc "Section V Student Evaluation " \l 2STUDENT EVALUATION Testing RequirementsNOTE: Describe how the student must demonstrate accomplishment of the TLO. Refer student to the Student Evaluation Plan.1. Performance Exercise: a. This task may be evaluated during a training exercise involving a MEDEVAC vehicle, or it may be simulated by creating a scenario and providing the information as the student requests it. The instructor will develop and provide the scenario information for the student to use. (A sample scenario that can be used or modified is located in the "Solution" portion of Appendix C.) The instructor and/or assistants will act as the radio contact at the evacuation unit during transmission of the request. b. Tell the student to prepare and transmit a MEDEVAC request. The student is given an order to request MEDEVAC, a patient pickup site, secure operational communications system, and the MEDEVAC request format. c. The student transmits a MEDEVAC request and provides all necessary MEDEVAC request information. d. The instructor and/or qualified assistants (as appropriate) will use the Performance Evaluation sheet located at Appendix C to evaluate the Soldier's performance. Score the student GO if all the steps are passed. Score the student NO-GO if any steps are failed. The performance exercise must be completed with 100% accuracy. e. The time required to accomplish this practical exercise is based on one instructor with 25 students, in a classroom environment. The size of the class, location of training, and availability of qualified assistants are relevant to time requirements. 2. Written Test: None.
