Your accruals will be charged for the first five days of your absence If you do not have accruals, you will be removed from payroll immediately The Statutory Benefit Program While on Workers' Compensation Disability Leave Without Pay (Section 71 of the Civil Service Law), you are considered to be in full pay status for the length of your disability for up to a maximum of twelve cumulative months for the following purposes: --health insurance, --accruing biweekly leave accruals, --continuous service, --retirement service credit, --Employee Benefit Fund contributions. You should be aware, however, that although you are considered to be in full pay status, payroll deductions you previously authorized cannot be taken while you are on leave without pay. This is because you will no longer receive a payroll check from this agency and deductions are not allowed to be taken from your workers' compensation wage replacement check under the Workers' Compensation Law. If you do not return to work within one year, your employment with the College will be terminated. Authorized Payroll Deductions To help you during your absence, we have compiled the following list of the most common deductions, what you should do to continue or discontinue them, and/or whom to contact if you have any questions. Retirement (Tier III and Tier IV Members) Upon return to the payroll, the Retirement System will automatically adjust the amount of your contributions to allow for deductions not taken during leave without pay. Health Insurance YOUR HEALTH INSURANCE CONTINUES AUTOMATICALLY Your Health Insurance Plan coverage will continue automatically while you are on workers' compensation leave for up to 12 cumulative months of disability at the same rate as an active employee. However, since you are no longer receiving a paycheck, you must make a choice as outlined below. You will receive a notice from the Department of Civil Service asking you to make this choice. Continue coverage, but defer payment until your return to the payroll. Choose Defer My Payments on the notice from the Department of Civil Service. When you return to work, your payroll deductions for health insurance will start again. In addition to the regular health insurance deduction, you will have a special deduction of up to $55 each paycheck to cover the health insurance premium for the time you were on workers' compensation disability leave without pay. The special deduction will continue until you have completed paying for the coverage you had while on leave. In the cases of extreme hardship, you may request a reduction to the $55.00 special deduction by writing to the Employee Benefits Division, Attention Workers Compensation Appeal. Continue coverage by continuing payments directly to the Department of Civil Service. Choose Pay Directly on the notice from the Department of Civil Service and attach a check in the amount of your deductions. Cancel your health insurance coverage: You may cancel your coverage when you go on workers' compensation leave by contacting the Office of Human Resources. Remember, cancellation would terminate your coverage for any non-work-related illness or injury and coverage for you and your family, if you are enrolled in Family Coverage. REMIND YOUR DOCTOR OF WORKERS' COMPENSATION AND DON'T SUBMIT CLAIMS UNDER NEW YORK STATE HEALTH INSURANCE PROGRAM. When you visit your doctor or other provider for services connected to your work-related illness or injury, be sure to tell your doctor that this is a workers' compensation-related visit. Your doctor or other approved provider, such as a physical therapist, will then submit claims to the State Insurance Fund. The State Insurance Fund pays for all necessary medical care for your work-related illness or injury. Therefore, for those visits, you are not responsible for any co-payments or deductibles that would otherwise be required. Do not file claims under your New York State Health Insurance Program (NYSHIP). The health insurance carriers are not responsible for your workers' compensation-related claims. You and your family continue to use the health insurance plan for all of your non-work related needs. REPORT CHANGES Keep your coverage up to date while you are on leave. If you need to change your health insurance coverage (from Family to Individual, for example), or if your child loses eligibility for coverage, they continue to be eligible for benefits under COBRA, the federal continuation law. There is a 60-day deadline for requesting continuation under COBRA -- contact the Office of Human Resources promptly. Credit Union Call or write to your credit union to make arrangements for making direct payment. CSEA Dues Contact CSEA's Membership Department at 1-800-342-4146 (at the prompt, dial 9 and then extension 1326) to have your CSEA dues waived for up to one year. CSEA Insurances If you are covered by insurances through CSEA and you have any questions regarding your coverage, please call CSEA at 1-800-342-4146. State Employees' Federated Appeal (SEFA) If you wish to fulfill your pledge, mail your contribution to the community campaign where your pledge was initiated. Checks should be made payable to SEFA and include the name and location of the employing agency. If you are uncertain where the pledge was initiated, the community campaign code is on the back of your SEFA pledge card. Payroll Savings Bonds Call National Bond and Trust Company (NBT) between 8 a.m. and 7 p.m. for information on savings bonds. Deferred Compensation Payments to the Deferred Compensation Plan can only be made by payroll deduction. Deferred Compensation will not accept direct payments toward this plan. Therefore, while you are on leave without pay, payments to the Deferred Compensation will not be made. Upon return to the payroll, however, you will be automatically reinstituted in the Deferred Compensation Plan. If you wish to discontinue participation or have questions on your account, please contact NYS Deferred Compensation directly at 1-800-422-8463.
