It outlines eligibility, nomination, and selection criteria. 1. POLICY: To establish procedures for selection of the 628th Air Mobility Support Squadron Airman (Amn), Noncommissioned Officer (NCO), Senior NCO (SNCO), Company Grade Officer (CGO), Civilian, Spouse, and Volunteer of the Quarter and Year. 2. TERMS EXPLAINED AND ELIGIBILITY CRITERIA: a. Annual: 1 January through 31 December. b. Quarterly: 1 January through 31 March (1st Qtr), 1 April through 30 June (2nd Qtr), 1 July through 30 September (3rd Qtr), 1 October through 31 December (4th Qtr). c. Airman: Airman Basic through Senior Airman (E-1 through E-4). d. NCO: Sergeant through Technical Sergeant (E-4 through E-6) e. Senior NCO: Master Sergeant through Chief Master Sergeant (E-7 through E-9). f. Company Grade Officer: 2nd Lieutenant through Captain (O-1 through O-3). g. Civilian: There are two categories; Civilian Manager (supervisory) and Civilian Specialist (non-supervisory). h. Spouse: An individual married to a 628 AMSS person. i. Volunteer: Any military member who volunteers to aid in base/local community activities. j. Selected winners would not be considered for nomination within the same category for a 12 month period (1 January through 31 December). k. Nominations for the annual awards are not limited to past quarterly award winners. 3. BASIC QUALIFICATION CRITERIA: a. MILITARY: Served in the appropriate grade for the category in which nominated for at least 2 months of the award period. b. MILITARY: Not have been convicted by court-martial or punished under Article 15 proceedings during or for 1 year preceding the award period. c. MILITARY: Does not have or had an Unfavorable Information File (UIF) during any part of the award period. d. MILITARY: Complies with Air Force personal appearance requirements. Individuals on the Weight Management Program can be nominated for an award only if they have been in Phase II, Observation Phase for two consecutive months or in the one year Probation Phase. e. MILITARY: Have an Air Force specialty skill level commensurate with their grade, or have demonstrated outstanding performance in their career field. f. CIVILIAN: Demonstrated outstanding technical and managerial skills in support to the squadron mission. g. SPOUSE: Demonstrated outstanding support, directly or indirectly, to the squadron and/or wing mission through volunteer efforts. h. VOLUNTEER: Nominations should consider the impact the individual made on the Incirlik community during the quarter they are being nominated for. 4. BASIC EVALUATION CRITERIA: a. Cited accomplishments must apply to the period for which the award pertains. However, specific achievements initiated before but culminated during the awards period, may be included in the nomination package. Quarterly award nomination packages, and required categories, will be prepared IAW Atch 1 and submitted IAW Atch 3. The electronic form version (Apr 95) of the AF Form 1206 is required. Limit the quarterly award nomination package to one typed page. b. ANNUAL AWARDS: Annual award nominees do not have to be previous quarterly award winners. Anyone can be submitted for annual awards provided they have not possessed any negative quality indicators during the award period. Squadron Annual Award winners will be forwarded to 621st as the squadrons nominations for 12 Outstanding Airmen of the Year and USAF First Sergeant of the Year awards. Recipients of this award will go on to compete at 21 AF, AMC, and USAF level. (1) Annual Award Nomination Packages for 12 OAY and USAF First Sergeant of the Year: Each package must contain the following: (a) The electronic form version (Apr 95) of the AF Form 1206 is required. (c) One-page biography. One original and eight copies (Atch 2c). (d) Base level records review report on individual personnel. One original and eight copies. (3) Annual Award Nomination Packages for Civilian of the Year: Each package must contain the electronic form version (Apr 95) of the AF Form 1206. Limit to two, single-spaced typewritten pages, one original and three copies. Accomplish the AF Form 1206 IAW Atch 2. There are four categories: (a) Employees in grades GS-01 through 08 and WG/WL-01 through 07 compete in category 1. (b) Employees in grades GS-09 through 13, WS-01 through 13, and WG- through WL-08 and above compete in category 2. (c) Local national employees in non-supervisory positions, in any grade, compete in category 3. (d) Local national employees in supervisory positions, in any grade, compete in category 4. (4) Annual Award Nomination Packages for Joan Orr Air Force Spouse of the Year: The 621 AMSG will provide a suspense date for this award, normally due in October of the current calendar year. Each package must contain the following: (a) The electronic form version (Apr 95) of the AF Form 1206 is required. Limit to two, single-spaced typewritten pages, one original and eight copies. Accomplish the AF Form 1206 IAW Atch 1c. (b) One-page biography. One original and eight copies (Atch 2c). (c) A citation, limited to 50 words or less (Atch 2g). 5. NOMINATION/SELECTION CRITERIA: a. Each flight (Command Section, MA, TR) can only submit one nomination package for each military/civilian award categories. There is no limit to any other nomination category. b. Nominations will be forwarded to the First Sergeant as indicated in Attachment 1 and 2. c. The Enlisted Boards will consist of three voting members (Senior NCOs) and the First Sergeant or a CMSgt as president. The First Sergeant will select the board members. Board members will be solicited from each flight. d. The Company Grade Officer, Civilian, Volunteer, and Spouse Boards will consist of the 628 AMSS/CC, CD, and CCE. e. The 628 AMSS/CC will notify the squadron within 72 hours after the board selection. Plaques will be forwarded to the winners within 30 days after releasing the results of the board. d. Upon completion of the board, selectees names will be submitted to the Commander through the First Sergeant for approval prior to public release. e. The nominees selected will be based on the nomination package. f. The Board President is a non-voting member except in the event of a tie. 6. UNIFORM REQUIREMENTS: Board members are required to wear their duty uniform. 7. RESPONSIBILITIES: All members of the 628th Air Mobility Support Squadron share the responsibility for a successful individual recognition program. Specific responsibilities are as follows: a. Supervisors will: (1) Ensure participation from assigned personnel in supporting award programs. (2) Identify and recognize outstanding performers. (3) Submit nominations in accordance with this letter. (4) Ensure nominees meet appropriate appearance and weight standards. (5) Support the First Sergeant in selecting board members. (6) Encourage maximum participation at the award ceremonies. b. The Board President will: (1) Contact the First Sergeant two days prior to the board to receive instructions regarding the ground rules and scoring process (Atch 4). (2) Provide the First Sergeant with the results of the board for the Commanders approval. (3) Ensure the board is conducted fairly. (4) Provide feedback to supervisors on nominees packages. c. The First Sergeant will: (1) Manage the squadron personnel recognition program (i.e. Amn, NCO, SNCO, CGO, Civilian, Spouse, and Volunteer). (2) Ensure selection board dates are confirmed or established and a room in which to conduct selection boards is reserved. (3) Ensure information on nomination formats and suspenses are provided to all sections. (4) Ensure all nominees meet the eligibility criteria established in squadron/group/base instructions or policy letters. (5) Coordinate the announcement of selectees. (6) Ensure all appropriate awards, plaques, letters, etc., are prepared. (7) Ensure packages are properly prepared in correct format and submitted on time to the group/wing for consideration. d. The Commander or designated representative will: (1) Announce the selection results to the squadron within 72 hours after the board selection.
