Overview The clinic network consists of a Wide Area Network (WAN) connecting the three remote clinic sites and a Local Area Network (LAN) located in the main clinic site. [INSERT NETWORK DIAGRAM HERE] The main clinic site holds the bulk of the administrative staff for the clinic, and contains the server room and communications equipment to connect all the sites. The clinic serves a large number of patients with a small IT staff and budget. The network has been built mostly with white box equipment, lower end processors, and outsourced email. The clinic has employed one staff person with an IT/IS role for the last several years, after first coming on board to help with the rollout of the practice management system. This role has expanded with the rapid growth of technology in use in the clinic Server Room This is a general diagram of the hardware in the server room. [INSERT SERVER ROOM DIAGRAM HERE] Wide Area Network The clinic has a WAN that connects the three remote sites to the practice management system server that is the core application supporting clinic operations. The main clinic site hosts the serves as the central point for these WAN connections. Currently the WAN is kept almost completely separate from the LAN.. The clinic has a leased line that connects the first remote site to the main clinic site. The leased line uses the Motorola UDS, AdTran TSU, and the Micom hardware in the server room in the main clinic site. The only problem the clinic has had with this is trouble with the phone line. To connect the second and third remote clinics to the main clinic site, the clinic uses uses Frame Relay lines. The practice management system vendor does not support these lines, though they are much less expensive to operate than the leased line. A PortMaster 2e Communications Server from Livingston Enterprises in the main clinic site, and a similar Com Server in the second and third remote clinics manage the frame relay lines. Livingston Enterprises is now out of business, though Lucent now maintains technical support information for this line at  HYPERLINK "http://www.livingston.com/tech/docs/manuals.html" http://www.livingston.com/tech/docs/manuals.html. Local Area Network The LAN is located in the main clinic site is used to connect the administrative staff to each other and the Internet. There are currently two servers in place, and old Netware 3.12 server, and an NT 4.0 server, although, only the NT Server is used with regular frequency. The Netware server is used exclusively for the accounting software system which uses the NPRINT command to print. NPRINT is a proprietary command and requires a Novell Netware Server. The main server is the NT 4.0 Server. This server is detailed in the NT Server section below, but is mainly just serving as a file and printer server, is running DHCP, and Seagate Backup Exec 7.0. The network is wired directly from the NIC on the client computers to three hubs located in the server room on the first floor, and two hubs located in the IT office on the second floor of the main clinic site. The hubs are 3COM OfficeConnect 10BT 16 port models, and have about 13 available ports. The network at the main clinic site is using Netbeui, IPX/SPX, and TCP/IP protocols on the network. NT Server The NT Server is providing basic file serving, printer sharing and DHCP. There are users and security settings on the network shares, however the clients are not logging into the domain, so the security is more or less moot. The passwords are set to never expire. The clinic is using the following groups Accounting Billing Clinic Development Education Executive Officer Finance Operations Grants Manager Human Resources Office Manager Public Affairs Shares There are extensive shares setup on the clinics network however these shares can be grouped into three major shares F:\Users\ Specific users have shares to the folders with their name. User and Admin has Full controlF:\CD-SBAdmin has Full ControlF:\AppsEveryone has full control, but some folders underneath have tighter security on a separate share Practice Management System The practice management system is an extremely complex system with many very specific problems. Because the clinic is currently in process of changing to a new system, it is not the intent of this document to go into detail on the system and troubleshooting steps. However do the complex nature of the system, this will serve as an overview and hopefully give you some idea of how it is configured.. The system is an old command-line driven client/server-based application. The Server software runs on an old IBM RISC 6000 C10 server located in the server room of the main clinic site. The server is UNIX based and has 512MB of RAM; it was upgraded recently to this level. The hard drive typically hovers at around 10% free, and IT staff has been purging six month old appointment data every few months to keep the server running at maximum performance. The server has its own UPS and two 14.4 modems connected for electronic filing and remote support. The terminals connect to the server through three black concentrators located just above the practice management system server. Most terminals (the ones wired during system setup) in the main clinic site connect to a patch panel, then plug into the concentrator, while others connect straight into the concentrator. Remote sites have concentrators that plug into the WAN, and then connect from the WAN device into the concentrator at the main clinic site. The majority of the terminals are IBM 30xx models. IT staff has installed some new purchases of IBM 31xx terminals recently without much trouble, though finding the terminals themselves was not easy. The PC workstations can connect to the practice management system through the use of PC Emulator software. The practice management system server must be backed up nightly. If there are any problems with the backup, users will be locked out of the system the next day. There is a tape backup drive on the server, and the tapes are stored on the shelf just above the server. Printers The network at the main clinic site has a number of printers, the majority of which are connected to the practice management system. These are primarily older dot matrix printers. In addition to these there are a number of local inkjet printers throughout the office. There are three printers served from the NT Server, an HP DeskJet 810C, an HP LaserJet 4050N, and a Toshiba DP6570. The backup jobs are scheduled to run using Seagate Backup Exec v7 rev 662. The backup type is Full Daily for the C, D, and F drives, with hardware compression. The tapes are labeled Monday-Friday and are stored on the shelf just above the server. There is no backup of the client computers; however, IT staff has redirected the default save locations to point to the network so the majority of users data is on the server. Internet Access The clinic has a 256K/64K ADSL line in the main clinic site and 3 Dial-Up accounts provided by Internet Service, which is primarily a local ISP. The network at the main clinic site is protected by a Cisco 506 PIX Firewall that is also performing NAT. There is a separate dial-up account for each office. IT staff have not had any problems with multiple logins using this account. The clinic is currently paying $335/month to Impulse for their internet connections. Contact them directly to set up new accounts, delete unneeded accounts, and receive technical support POP3 clinic01.clinic.org SMTP mail01.clinic.org Outlook Express is the supported client for users to access their email. Web Server The clinic has a website that is maintained by the Public Affairs group. The technical staff has no roles or responsibilities in the upkeep or maintenance of this site currently. The site is located at http://www.clinic.org The NT Server is running IIS 3.0, however only the default sample pages are loaded.
