I understand if I was completely unsuccessful my most recent term enrolled, I need to speak to a Financial Aid staff member regarding eligibility.

I understand if I have attempted more than 96 credit hours, I must visit with the Financial Aid Office before enrolling in classes. If I exceed my maximum time frame, I am not eligible for federal aid.

I understand I must provide my official high school transcript or GED and any college transcripts to the CCC Registrar's Office. Failure to do so will impact my financial aid.

If my account has a credit balance, Coffeyville Community College will be provide me with a refund of the balance. I understand that my aid will be disbursed based on my certified enrollment status and may be different from my award amounts. All awards are split into two disbursements, one for fall and one for spring, unless you are graduating in the fall.

Loan amounts are limited by federal loan limits, other aid you may be receiving, and your financial need. You must enroll in at least 6 credit hours per semester in an eligible degree or certificate program to receive a loan. The actual amount of your awar d will be determined by the Financial Aid Office.

* Dependent students may qualify for the additional unsubsidized loan amount as a r esult of the par ent being denied the federal direct parent plus loan. Your parent must visit to apply.

THE FOLLOWING STEPS MUST BE COMPLETED

1. Have your FSA ID and Password that you used to sign the FAFSA. FORGOT IT? Go to fsaid. 2. Login in to with your FSA ID MUST HAV E ID TO LOGIN 3. Choose ENTRANCE COUNSELING on "My Homepage"

Add CCC in the "Notify School" section Choose "I'm completing entrance counseling as an undergraduate" Follow instructions as you go through each page. Master Promissory Note 1. Click Complete a Master Promissory Note (MPN) in the middle of the page. You may also go back to your "My Homepage" and choose MPN there.

Type of loan you are applying for is : Subsidized/ Unsubsidized A temporary school address is unacceptable. Must provide 2 complete references (Name, Address, Phone) that do not live at the same address. Read Terms and Conditions ** VERY IMPORTANT: Make sure you electronically sign the MPN by typing in your First name, middle initial, and last name.
