___ Copy of Birth Certificate (NEW STUDENTS ONLY) DUE BY AUGUST 15th

___ Photo Release Form DUE BY AUGUST 15th

___ Signed Parent Handbook Acknowledgement Form DUE BY AUGUST 15th

___ TWO sets of clothes clearly labeled in a Ziplock bag (shirt, shorts, socks, climate appropriate). Please label individual clothing items as well. DUE BY AUGUST 15th

___ If your child is in diapers: ONE small package of diapers (or about 20 diapers) and ONE small package of baby wipes, both labeled with his or her name. DUE BY AUGUST 15th

___ If your child is in underwear: THREE pairs of underwear and THREE bottoms both labeled with their name (in addition to the two sets listed above). DUE BY AUGUST 15th

___ Classroom Slippers: Foamtreads: Can also be found on Amazon. Only purchase solid colored slippers. DUE 1ST DAY OF SCHOOL

___ ONE box of gallon Ziplock Bags DUE BY AUGUST 15th

___ TWO containers of disinfecting wipes (ie: Lysol or Clorox wipes) DUE BY AUGUST 15th

___ ONE family photo to be displayed in the classroom (not to be limited to just parents or siblings, but anyone your child considers important, i.e.: pets, other relatives). DUE BY AUGUST 15th

___ ONE small clear zippered cosmetic bag and ONE small hair brush that will fit into cosmetic bag. Please refrain from characters on the bag and brush. DUE BY AUGUST 15th

** ANY STUDENT WITH MISSING PAPERWORK, NOT REGISTERED THROUGH FACTS TUITION MANAGEMENT, OR WITH AN ACCOUNT BALANCE WILL NOT BE ABLE TO START SCHOOL UNTIL ALL PAPERWORK IS ON FILE, FACTS ACCOUNT IS ESTABLISHED AND ACCOUNT BALANCES ARE RESOLVED.
