Terms and Conditions

1. The Aurora Pharmacy Prescription Savings Club is not an insurance program, a Medicare prescription drug plan or health insurance policy. The Prescription Savings Club is a prescription drug buying program for individuals who have no prescription drug coverage, or insufficient drug coverage. Persons receiving benefits from a publicly funded health care program are ineligible for the Prescription Savings Club program. As a participant, you are entitled to discounts on certain prescription drugs at any Aurora Pharmacy. The Aurora Pharmacy Prescription Savings Club does not entitle you to any discounts or benefits at any other pharmacy and may not be used in conjunction with insurance, other discount programs, or other prescription benefits. Club benefits are subject to change, modification or substitution without notice.

2. The savings club applies only to certain drugs at commonly prescribed dosages. You may obtain a list of the generic drugs covered under the savings club at any Aurora Pharmacy or at . Not all formulations of a drug are covered under the savings club.

3. Savings club members must pay an annual enrollment fee of $5 upon fulfillment of the first prescription under the savings club. No refunds will be made after the first prescription is fulfilled using the savings club. Membership is non-transferrable. Only active members of the savings club may participate. Under the savings club, $3.99 is the price for up to a 30-day supply of certain covered

generic drugs at commonly prescribed dosages. $9.99 is the price of a 90-day supply of certain covered generic drugs at commonly prescribed dosages. Certain generic drugs indicated with an asterisk(*) will be specially priced ranging from $6.99 for a 30day supply up to $24.99 for a 90-day supply. Prices are subject to change.

4. Prescriptions must initially be filled in person and there are no substitutions.

5. You understand that your prescription medications are dispensed only in accordance with the order of your physician or other health care provider. Membership in this program does not alter or affect your ability to obtain prescription medication or your responsibility to use prescribed medication in accordance with the directions of your physician or other health care provider and your Aurora pharmacist.
