Birthday Party Details

Liability Guidelines

In consideration of this application for memberships, birthday parties and all other events, I or my child, release Kevin & Jamie Sousa/Abilities Rec, Inc., its employees, agents, representatives, and other persons or organizations for whose conduct it is responsible from any and all liability, loss, damage, cost, claims and/or causes of action, including but not limited to all bodily injuries and property damages resulting or arising from the use of premises, facilities, or equipment of Abilities Rec, Inc., or caused in any way by Abilities Rec, Inc. employees, agents, representatives, and other persons or organizations for whose conduct it is responsible. I and/or my child are in the necessary physical condition to participate in the registered activity. I authorize the staff to seek emergency medical care on my behalf or my child if needed. I will assume all costs. I also give Abilities Rec, Inc. permission to use my child's photo, without posting name or age, for promotional purposes of our program only.

Payment and Cancellation Policies

All pricing and rates are subject to change. The birthday party base rate is $300.00. Parties include: *2.0 hours (75 minutes of pre-selected programming and 45 minutes in the party room) *Access to the Sensory room *Private party, not open to the public *up to 15 kids including the birthday child *All paper goods and balloons supplied *small bottle of water and gift bag item for each participant *At least two party hosts to entertain *Outside food and drinks welcome (refrigerator and freezer available) Additional fees may apply, based on the party details provided above. Payment is due before or on the day of the event. If additional children attend, or additional time is requested, Abilities Rec, Inc. may adjust the total cost accordingly. Abilities Rec, Inc. will accept payment via, cash, check or debit/credit card. If a party is cancelled 3 or more days prior to the booked date, you will receive a full refund. If the party is cancelled within 2 days of the event, a $75.00 cancellation fee will apply. In the unlikely event that If the event is cancelled by Abilities Rec, Inc. you will be given the choice of full credit or a full refund.

Event Policies and Timeline

1. Hosts are welcome to arrive any time within 15 minutes of your scheduled start time. 2. You are welcome to bring your own cake and ice cream or other outside food. 3. Abilities Rec, Inc. will provide two party coaches to facilitate the fun, assist with room set up and take

down, tables, seating, and table cloths. You are welcome to bring your own decorations, however please do not bring glitter or confetti. An additional cleaning fee may be charged for glitter/confetti.
