Section 1 Introduction

This directory provides a list of HMSA Akamai Advantage network providers and pharmacies effective Jan. 1, 2022. To get detailed information about your health care coverage, please see your Evidence of Coverage. To get a complete description of your prescription coverage, including how to fill your prescriptions, please review the Evidence of Coverage and the HMSA Akamai Advantage's formulary.

Network Providers

As of September 24, 2022, our network includes 4297 practitioners, 165 facilities, and 215 pharmacy locations. You may choose one of our network providers listed in this directory to be your primary care provider (PCP). The term "PCP" will be used throughout this directory.

Generally, you must get your health care services from your PCP. Your PCP is a health care professional who meets state requirements and is trained to give you basic medical care. Your PCP will also coordinate the rest of the covered services you get as an HMSA Akamai Advantage member.

Although HMSA Akamai Advantage members do not need a referral to see a specialist, you should ask your PCP to refer you to an HMSA Akamai Advantage network provider. If an HMSA Akamai Advantage network provider is not available, your PCP may refer you to an outof-network provider.

PCPs are listed in Section 3 of this directory and at our website, . You should contact our Customer Relations department for help in choosing or changing your PCP. Their telephone numbers are listed on the front cover of this booklet.

The network providers listed in this directory have agreed to provide you with your health care services. You may go to any of our network providers listed in this directory. If you have been going to one network provider, you are not required to continue going to that same provider.

In some cases, you may get covered services from non-network providers. However, your out-of-pocket expenses may cost more. In most cases, your health care services are covered under HMSA Akamai Advantage.

If you're a member of HMSA Akamai Advantage Dual Care, your health care coverage is covered under both Medicare and Medicaid. In this directory, each of the providers and facilities that participate in both Medicare and QUEST Integration are marked with a "" sign. This sign lets you know that when you use this provider or facility, your costs will be covered.

You may go to other in-network or out-of-network providers, but may incur additional out-ofpocket expenses. For details about your HMSA Akamai Advantage Dual Care health care coverage, please see your Evidence of Coverage.

Network Pharmacies

This booklet provides a list of HMSA Akamai Advantage's network pharmacies. To get a complete description of your prescription coverage, including how to fill your prescriptions, please review the Evidence of Coverage and HMSA Akamai Advantage's formulary.

We call the pharmacies on this list our "network pharmacies" because we have made arrangements with them to provide prescription drugs to plan members. In most cases, your prescriptions are covered under HMSA Akamai Advantage only if they are filled at a network pharmacy or through our mail-order pharmacy service.

Once you go to one pharmacy, you are not required to continue going to the same pharmacy to fill your prescription but can switch to any other of our network pharmacies. We will fill prescriptions at non-network pharmacies under certain circumstances as described in your Evidence of Coverage.

All network pharmacies may not be listed in this directory. Pharmacies may have been added or removed from the list after this
