AD Checklist for Coaches Obtaining 5 Year Coaching Certificate

If CPR Certification has expired, please renew.

This link is for the CPR course: (separate from the AESOP course)

Once these courses are completed, please submit copies of the 3 certificates OR send the transcripts showing that you completed them to the District Office of Athletics. Also send a front and back copy of the CPR card that is signed.

Once you have completed these 3 courses along with the CPR course, you must apply to the FLDOE's website for your 5 year athletic coaching certificate.

After all the above has been completed you can begin coaching and working with the student athletes of Polk County. Supplements will be prorated by weeks until completion.

The Polk County School District is excited to introduce a coaching education and certification program that is delivered online

and accepted by the Florida Department of Education.

The Polk County School District in conjunction with leading coaching education provider Human Kinetics Coach Education offers high school coaches the opportunity to take the online courses listed below to meet the Florida Department of Education Athletic Coaching Certification and Endorsement. Certification is earned by successfully completing the required courses with a passing score of 80 percent or better.

Courses that are convenient, affordable, and effective Coaches will benefit from the anytime-anywhere convenience of online learning. With these courses, coaches have the flexibility of completing course segments when and where it's convenient for them.

ASEP online courses offered for certification

Drawing from the text Successful Coaching, written by ASEP founder Rainer Martens, Ph.D., Coaching Principles provides coaches with essential instruction on the principles of coaching, behavior, teaching, physical training, and management. Coaching Principles is the study of the fundamentals of being a successful coach--before even stepping on the playing field.

Through this course, students are challenged to: ? Define who they are as coaches (their coaching philosophy,

objectives, and style); ? Enhance communication and motivational skills; ? Become more effective teachers and trainers; ? Improveteam,relationship,and risk managementskills.

According to National Athletic Trainers Association statistics, nearly half of sports injuries occur during practice. Sport First Aid is designed to help coaches learn the skills to be a competent first responder to players' injuries and illnesses. Sport First Aid is different from general first-aid courses in that its focus is on dealing with the injuries and illnesses a coach can expect to encounter on the playing field:

? Develop basic knowledge of sport injuries; ? Recognize common sport injuries; ? Administer appropriatefirst aid.

From these online courses, coaches gain a solid understanding of sportspecific technical and tactical skills in order to teach these skills effectively. SEND COMPLETED FORM ALONG WITH COPIES OF PROOF OF COMPLETED ONLINE COURSES AND CPR CARD TO DON BRIDGES, DISTRICT OFFICE.

INSERVICE POINTS WILL THEN BE AWARDED AND COACH PROVIDED WITH ONLINE INFORMATION TO APPLY TO DOE FOR A FIVE-YEAR COACHING CERTIFICATE.

POLK COUNTY SCHOOLS APPLYING FOR A COACHING CERTIFICATE

Apply for your Athletic Coaching Certificate online at the Department of Education (DOE). This application requires payment with a VISA or MasterCard.

1. Access the DOE website 2. Log In or Create Account. Use Options if necessary. Be sure to complete this section

and write down your User Name and Password. (You may receive an email from DOE with a temporary password to use initially.) 3. Click on Submit Application 4. Read instructions and accept user agreement 5. On the New Application screen Choose Athletic Coaching under Action Requested and click Add 6. Click Continue 7. Skip over the following sections: Non Fl Cert & Teaching Experience and Click Continue 8. Skip Academic Training if not applicable 9. Answer all questions on the Sealed/Expunged Record(s) - Step 1 of 3 10. IMPORTANT: Click Save to insert your Criminal Offense response and then click Continue when the screen reappears 11. Answer all questions on the Criminal Offense Record(s) - Step 2 of 3 12. Click Save and then click Continue 13. Answer all questions on Professional Sanction - Step 3 of 3 14. Click Save and then click Continue 15. Important ? Verify that all information is correct on Affidavit and Payment. Edit as necessary. 16. Important ? Be sure you applied for Athletic Coaching under Process and Subject(s) Requested is Athletic Coaching ? (Grades K-12) 17. On Applicant's Affidavit, you must check each box before clicking Continue 18. Review Summary of Pre-Fees, read information and if correct check the box in front of your name. 19. Click Continue 20. IMPORTANT: Send a copy of the Online Return Letter to the District Director of Athletics to verify you have completed this procedure.
