Dear Parents

Applications for Grade 8 2019 will be done on the Gauteng Department of Education's website ? .za (The date for the opening of Admissions is still to be confirmed by the GDE). Please keep an eye on the website for updates and ask to be added to our mailing list.

NB: As waiting lists are drawn up on a strict first-come-first-served basis, it is imperative that you submit your application promptly! There are a limited number of spaces available and the submission of an application does not guarantee placement even though your application may meet all the admissions criteria.

On submission of the online application, you will receive a waiting list number from the GDE. You will also be notified that you have a certain number of working days in which to submit your documentation to the school for verification. Documents may be submitted between 07h00 and 13h00 daily. If you fail to submit your documentation to the school within the required number of days, it will result in your application being unsuccessful.

The following documentation will be required:

CERTIFIED COPIES OF:

1) Your child's unabridged birth certificate 2) Your child's latest school report 3) Both parent's ID's 4) Your proof of residence 5) Proof of employment for both parents

NB: Certified copies may not be older than 3 months.

DOCUMENT REQUIREMENTS EXPLAINED:

? Unabridged birth certificate ? This version also reflects the parents' details. If you do not have this version, please apply for one at Home Affairs. For the purpose of submitting documentation to the school, you may submit the birth certificate that you do have, together with a receipt from Home Affairs confirming that you have applied for the unabridged version. Once you have received the unabridged version from Home Affairs please submit it to the school.

Proof of Residence:

? If you are renting please supply a certified copy of a current up to date lease agreement (reflecting any extensions to the lease period) and attach it to a completed Affidavit ? (Annexure A which can be found as a separate document on our website).

? If you are living with your employer or a member of your family, it is necessary for that person to complete an Affidavit - Annexure B (which can be found as a separate document on our website). This then needs to be attached to their proof of residence (Lease or Utility Bill).

? If you own your own property and the utility bill is in your name it is not necessary to complete either Annexure A or B. Please supply only the original or certified copy of the Municipal Bill reflecting your name and physical address. This utility bill may not be older than 2 months.

Proof of employment:

? Proof of employment is required for both parents in the form of a current pay slip or current letter from the HR Department.

? If you are self-employed please submit a letter from your accountant/bookkeeper together with your company registration papers.

Other requirements if applicable to your situation:

? Divorce agreement indicating custodial parent.

? As only a parent or LEGAL GUARDIAN may enrol a child at the school, it is necessary to provide proof of legal guardianship in the form of a COURT ORDER granting guardianship to the parties of this application, if you are not the biological parent of the child.

Immigrants:

? Certified copies of permanent residence or a valid visa for all parties of this application. No foreign student will be admitted to Fourways High unless they are in possession of a valid study visa for Fourways High School for the duration of their studies. Please note: The annual school fees for foreign students are required to be paid in full before the student commences their studies.

NB!! The provision of falsified information will lead to legal action being taken against the applicant and possible de?registration and consequent placement at another school.

IF YOUR APPLICATION THROUGH THE GDE IS SUCCESSFUL, YOU WILL BE NOTIFIED BY THE GDE VIA SMS, AS WELL AS RECEIVE AN EMAIL ACCEPTANCE FROM THE SCHOOL. THIS USUALLY HAPPENS JUST BEFORE THE END OF TERM TWO (22 JUNE) OR THE BEGINNING OF TERM THREE (17 JULY).

If you accept the offer, ONLY THEN will you be required to complete and submit the Fourways High On-line Data Capture Form. (This is the school's own application). PLEASE DO NOT DO THIS BEFORE YOUR CHILD HAS BEEN ACCEPTED!

You will be able to access this document via a link on our website or at scadco.co.za (Please refer to the available users-guide on our website). This document requires more detailed information and all fields must be completed.

You will receive a date and time stamped reference number on successful submission of this document. PLEASE TAKE CARE TO APPLY FOR 2019 AND NOT 2018.

? Once you have submitted the FHS online application, you are required to print out the completed form. Please print each page and ensure that the format is correct (A4 portrait).

? Both Parents/Legal Guardians are required to sign EACH page of the application form irrespective of marital status. Please contact me if you have any questions regarding this requirement.

PLEASE THEN SUBMIT THE SIGNED APPLICATION TO THE SCHOOL BETWEEN 07H00 AND 13H00.

A deposit of R3500.00 will be payable on acceptance to the school (for budgeting purposes please note that this is usually by the end of July). This fee goes towards the 2019 annual school fees, which will be set in August/September 2018.
