Organizational Health Survey

As you know, this is an exciting time in the evolution of our respective organizations. We are confident that we are bringing together two great organizations along with rich histories and many talented leaders, professionals, clinicians, supporters and more. To prepare ourselves for the journey ahead, we have engaged McKinsey & Company to assist us in our integration planning as well as the shaping of our new, combined culture. We sincerely request your assistance in our efforts by taking part in our confidential Organizational Health survey, available at . This survey will be open for physicians from February 22-March 7.

Please know that this is neither an engagement nor a satisfaction survey. In fact, this measurement tool is designed to assess strengths and opportunities of our respective organizations so that we can best determine where we may already be aligned and where we may have differences or gaps to address. The Organizational Health survey is a scientific approach to understanding culture based on in-depth research into what drives high-performing organizations over time. The results will provide greater insights into major management practices and behaviors, which will help us think about the "why" behind our organizations' cultures.

We believe it is critical for us to develop a strong, unified culture for the new Hackensack Meridian Health by building upon the best of both of our organizations. The feedback we garner from this survey will be used to help inform the future state of the new organization.

Please know that all responses will remain confidential and no one from either organization will be provided with or have access to individual results. The online survey will be facilitated and tabulated entirely by McKinsey & Company, and survey insights will be shared only in the aggregate in order to spark conversation around strengths, opportunities and vision for HMH.

The physician survey should take approximately 10 minutes. We encourage you to complete it during a quiet period when you can concentrate and provide thoughtful responses without interruption. We also recognize that sometimes the survey language may not appear as though it completely applies to you. We ask that you simply do your best to reply to the intent of the question.

As an added incentive, all those who participate will be entitled to a free pair of ear buds touting the new Hackensack Meridian Health logo. They can be picked up for your local hospital Medical Staff office. Participants will also be entered into a special drawing for lunch with both of us, the co-presidents of our future organization. What is the purpose of the Organizational Health Survey (OHS)? 1 We are committed to developing a strong, unified culture for the new Hackensack Meridian Health (HMH) entity and we think a great place to start is by building on the best of our organizations. We are administering this survey to help us better understand our common strengths and opportunities so that we can successfully integrate cultures.

What exactly is an Organizational Health Survey?

The Organizational Health Survey is a scientific approach to understanding culture based on in-depth research into what drives high-performing organizations over time. It is different from regular engagement or satisfaction surveys that you may have taken in the past in that it provides more insight into major management practices and behaviors.

Who is administering the survey?

We have engaged McKinsey & Company as our integration consultant partner. They will administer and tabulate this confidential survey on our behalf.

Is the survey confidential?

Yes, the survey is confidential. No one from either HackensackUHN or Meridian Health will have access to individual answers. Results reported will only include aggregates and percentages, and to ensure anonymity, no results will be shared for groups or divisions of fewer than 10 people. Additionally, any comments provided in response to open-ended questions will be grouped together and randomized before any results are shared.

If the survey is confidential, why do I have to enter a code, e.g. my employee or team member ID?

Your ID enables McKinsey & Company to look at similarities and differences for various groups across our organizations. They have already captured key information about each participant, such as job type, department and location which will be tied to your individual code. This information will only be reported at the group level; no one at either organization will see individual answers. Also, if you do not finish the survey in one sitting, your ID code will allow you to return to complete it at a later time.

How and when will results be used and shared?

Insights from this survey will be used to help inform the future state of the new organization. Survey insights will be shared initially with leadership in late March and more broadly with the organizations shortly thereafter to continue the conversation about our strengths, our opportunities, and our vision for the new HMH culture.

How can I access the survey?

Here is the survey link: . It is also being distributed via email and postcard mailings, and will be available on our organizations' intranet, at communications centers, on kiosks and on screensavers, and in whatever other manner is appropriate at each site and for each audience. You may access the link from any computer at your work site, or from your personal computer (however, your managers have

been asked to allow you time at work to complete the survey). Some participants will also have access to paper surveys if necessary.

Is the survey available in Spanish?

Yes, the survey is available in both English and Spanish.

How long will I have to complete the survey?

The survey will open for leaders (managers and above) from February 15 until February 29. The survey will open for front-line team members/employees from February 22 to March 7 and also for all physicians from February 22 to March 7. Each group will have two weeks to participate.

Do I have to complete the online survey in one sitting?

While completing the survey in one sitting is preferable so that you can remain focused and thoughtful, you will have the option to return to the survey at a later time to finish where you left off.

What should I expect from the survey?

Surveys for leaders (which include managers and above) will take approximately 20 minutes to complete. It is mostly multiple-choice questions, with two optional open-ended questions at the end.

Front-line team members will receive a survey that is half this length and will take 10 minutes. To reduce the time required for front-line team members to complete the survey, each team member will receive randomly assigned questions so they should not be concerned if they recognize that they answered different questions than their colleagues did.

Employed and independent physicians are also being given a shorter, 10 minute survey

Is there any incentive or drawing for participation?

Yes. Because we are so appreciative of your time and we really want high participation, all those who participate will be entitled to a free pair of ear buds touting the new Hackensack Meridian Health logo. Participants will also be entered into a special drawing for lunch with both of the co-presidents of our future organization, Robert Garrett and John Lloyd. You can watch for additional announcements at your site about the details of picking up your ear buds and the winners of the drawing.

Will it really matter if I participate?

The leadership of our respective organizations really want to get this right. The merger of our great organizations is a significant milestone and we are inviting all those that we employ as well as our medical staffs to voice their thoughts and opinions early. We want to capture the best of organizations and identify any misalignments so that we can collectively create the best future state for Hackensack Meridian Health.
