will result in the normal fee being doubled.

INSTRUCTIONS

1. This pre-printed form must be used in order to obtain a current vehicle sticker.

2. Return the entire form (do not separate) in the envelope provided with the appropriate fee. Make your check payable to "Village of Glendale Heights".

3. ADD FOLLOWING PENALTIES TO COST FOR LATE PURCHASE OF A VILLAGE STICKER July 1st-31st.........................$10.00 Aug 1st-31st.........................$20.00 Sept. 1st- May 1st................$30.00

4. A vehicle owner has 45 days from date of purchase or establishment of residency to purchase a vehicle sticker without penalty.

5. Transfer stickers are $1.00 when transfer is from original vehicle. Proof of original purchase must be presented.

6. Mail early to avoid last minute rush or late fees.

PER VILLAGE ORDINANCE PARKING ON VILLAGE STREETS IS PROHIBITED FROM 2:00 A.M. TO 6:00 A.M. UNLESS THE VEHICLE DISPLAYS A GLENDALE HEIGHTS VEHICLE STICKER

THE VILLAGE HAS THE RIGHT TO WITHHOLD ISSUANCE OF YOUR VEHICLE STICKER IF YOU ARE DELINQUENT AND FINANCIALLY INDEBTED TO THE VILLAGE.
