Center for Health Systems Innovation (CHSI), an

Vision and Mission

The Center for Health System Innovation (CHSI) will be an international leader in the development, evaluation and implementation of new methods that improve access to and the quality of chronic and acute health services interventions in all aspects of disseminated care delivery including the maintenance of wellness. CHSI will be housed within Keck Medicine of USC to maximize the translation of innovation into sustainable outcome improvements in the USC Health System. The CHSI will report to the Vice President of Research of USC, for the purpose of engaging faculty, staff and students throughout USC in CHSI’s mission.

Initially, it is expected that CHSI will focus on these topics:

• Create a Learning Health System in which continuous quality improvement and systems innovation are stimulated and reinforced at all levels of the organization

• Implement integration of research projects into daily operations; engage collaborative interaction between health system professionals and (future) health operations administrators/researchers

• Obtain research funding for process/quality improvement and healthcare/systems outcomes projects

• Maintenance of health system improvements stimulated by the ongoing Huron consulting engagement R3 (Right Care; Right Resources; Right Cost) and observation and research into the key factors required to sustain health system improvement

• Foster national and international collaboration with leading institutions to ensure USC’s prime position in this developing field

• Develop Fellowships and other educational tracks for graduate students in Medicine and University disciplines affiliated with the center

The CHSI is expected to attain measurable improvements in defined areas at Keck Medicine. Our goal is also to create an internationally renowned center for innovation in health systems through publication, sponsored research, outreach, industry partnerships, and technology transfer.

Director and Organization

CHSI will be led by Dr. Carol Peden, who will be a faculty member of the Department of Anesthesiology Professor of Anesthesiology-Clinical Scholar in the Keck School of Medicine. Dr. Peden is an international authority in improving quality and outcomes of care.. She has extensive experience of working in the tightly regulated and cost constrained National Health Service delivery model of care, but has also studied some of the best care delivery models in the US and around the world in her role as an Institute for Healthcare Improvement Fellow and Faculty. She spent a year at IHI in Boston 2008-9 and has subsequently worked on improvement programs in Scandinavia, the Middle and Far East, as well as throughout the UK. She has extensive experience of delivering both large and small-scale improvement projects and is a trained researcher with, in addition to her medical qualifications, a higher research doctorate and a Harvard MPH in Clinical Effectiveness. In her role as center director, she will report to the Vice President of Research; in her role as Professor in a University clinical department, she will report to the Keck School of Medicine Anesthesiology Department Chair. Initial Center staffing will include a project specialist to support the administration and execution of projects as well as a data analyst.

The center will engage faculty, students and staff from throughout the university by:

• Leading and facilitating faculty led sponsored research projects that utilize Keck Medicine and other aligned health systems as research laboratories.

• Placing students in paid internships as well as assisting in the development of experiential learning projects.

• Helping in the creation of new interdisciplinary training and degree programs in healthcare quality.

A central theme of CHSI will be turning research and education into sustainable improvement. The CHSI will develop strong linkages with the schools that provide health and wellness care, particularly the Keck School of Medicine, the School of Pharmacy and the Ostrow School of Dentistry. In addition, alliances will be formed in these areas:

Healthcare Communication: Annenberg School of Communication, School of Cinematic Arts and Institute for Creative Technologies

Quality Programs, Measurement, Simulation and Analytics: Viterbi School of Engineering, Marshall School of Business, Gould School of Law and School of Social Work.

Healthcare Technology and Information Technology: Viterbi School of Engineering

Economics, Policy and Administration: Price School of Public Policy, School of Pharmacy, Gould School of Law and Dornsife College of Letters, Arts and Sciences.

Population Studies and Prevention: Davis School of Gerontology and Keck School of Medicine Department of Preventive Medicine.

Education: Rossier School of Education, Keck School of Medicine, Dornsife College of Letters, Arts and Sciences - Department of Anthropology

Relationship to Existing Centers at USC

All of these centers have a mission of improving health, but none has a specific focus on health system innovation, and none has the dual role of conducting research and ensuring its translation and effective implementation within USC’s health system.

In addition, CHSI will develop an alliance with LA County Department of Health Services (DHS) to enable and facilitate collaborative healthcare improvement projects designed to enhance the care of Los Angeles County’s indigent and challenged populations in a cost effective manner while ensuring improved outcomes and access to care.

These and other faculties are expected to become affiliates of CHSI, meaning they will participate in CHSI projects while retaining their existing academic homes.

Sources of Funding

Core funding for CHSI, which provides for the director salary and salary of two staff members and basic office expenses will come from Keck Medicine of USC via ACC charges. None of these costs will be contributed by participating schools. In addition, space will be provided at no charge by Keck Medicine of USC.

Subsequent funding for the Center is anticipated from Center developed research grants (Focus on: Continuing Quality and Process Improvement; Health Systems Outcomes; Patient Satisfaction and Health Maintenance Education; Innovation in Systems Based Healthcare Delivery), Keck Medicine and Health System performance based incentives, and Philanthropy.

CHSI will supplement its funding through sponsored research from federal agencies (e.g., AHRQ, PCORI, CMMS Innovation Center, DOD and NIH), private foundations (e.g., Pew Foundation, Commonwealth Fund, California HealthCare Foundations) and state and local government (e.g., LA County Department of Health Services, California Department of Health Care Services).

Sponsored research funding will be distributed in accordance with USC’s memo on distribution of overhead, meaning that satellite accounts will be established within the schools of faculty representing their contributions to research projects. However, CHSI will be treated in the same manner as a school for work conducted by the CHSI director and CHSI staff. This means that overhead associated with the CHSI staff will be accrued by CHSI, and taxed by the university at the same rate as a school would be taxed. The net overhead will be applied toward the administrative costs of running CHSI.

CHSI reports to the Vice President for Research. It will establish: 1) a faculty advisory committee to guide the center in development of research programs and 2) an external advisory committee representing implementation sites, including Keck Medicine of USC and LA County Department of Health Services.

The Chief Administrative Officer of USC Keck Medicine will serve as the health system’s liaison to the CHSI to facilitate communication and implementation.
