D-20 APPLICATION INFORMATION

Examination for Certificate of Fitness for Fire and Emergency Drill Conductor for NYC Pre-K to 12 Nonpublic Schools (D-20)

D-20 can be obtained by qualifying for an exemption on the basis of education and experience. D-20 is only issued via Alternate Issuance Procedures (AIP) submitting the required documents. Applicants who do not qualify MUST take the computer based FDNY administered examination and obtain an F-07/W-07 Fire and Non-Fire Emergency Drill Conductor Certificate of Fitness.

Applicants must understand that the Fire Department reserves the right to randomly call up to test the applicants who are issued by AIP. Applicant who has failed the written F-07/W-07 exam will not be allowed to take advantage of this policy.

Alternative Issuance Procedure (AIP) Information for D-20 Applicants must be at least 18 years of age and must have a reasonable understanding of the English language and must follow the required procedure and submit the required materials.

1. How to apply:

If you meet the requirements below you can submit an application through FDNY Approved Designated Coordinator (DC) of the nonpublic school agency by mail. To request DC approval, email: Debbie.Rodriguez@fdny. or Elsa.Araya@fdny.

All of the required documents and payment listed in "2. Application Requirements" must be submitted via mail to:

If all required documents and payment are submitted correctly. The Certificate of Fitness will be mailed out to the school mailing address within 10 business days.

2. Application Requirements:

2.1 Education, License, Experience or Other Qualification Requirements (1) Training Certification/Letter attached with the applicant's graduation exam (answers and passing grade).

A certificate or letter on an FDNY approved training company/institution's official letterhead stating that the applicant has participated in and completed the Fire and Emergency Drill Conductor for NYC K-12 Nonpublic Schools (D-20) and General Response Protocol (GRP) training course and receiving a passing score of

at least 70% . Each applicant's graduate exam (recording their answers and final score) must be also attached.

(2) Employer's recommendation letter stating the applicant met one of the following qualifications: A Principal, Assistant Principal, School Director, Head of School; OR Pedagogical Staff who has supervisory or leadership responsibility to guide the school's curriculum and to co-administer the school.

Each applicant MUST submit the recommendation letter signed by the employer. Principal's recommendation letter must be signed by a superintendent or board member. Other staff's recommendation letter is allowed to be signed by the principal, superintendent or board member. This recommendation letter must be with school letterhead or notarized (sample letter is attached in this document).

Other applicants who do not meet the requirements listed above must obtain the F07/W-07 by passing the C of F exam at FDNY Headquarters.

A recent photo (2x2 head shot) in JPG or JPEG format name with applicant first and last name in a CD/flash drive or email to Debbie.Rodriguez@fdny. or Elsa.Araya@fdny.

2.5 Application Fee The application fee for D-20 is $ 25. The payment should be submitted by personal or company check or money order, payable to the New York City Fire Department.

3. Other Related Fees

3.1 To change a mailing address: Submit a letter requesting the change of mailing address and a copy of your C of F with $5.00 fee.

3.2 To change a work location, Submit a letter from your current employer (on company letterhead) confirming that you are an employee and stating your new work location with a copy of your C of F and a $5.00 fee

3.3 To request a replacement certificate: Submit a driver's license or passport, social security number, mailing address and a $5.00 fee.

4. Renewal Information

D-20 Certificates of Fitness are valid for three (3) years. The renewal fee is $15.The designated coordinator shall notify the FDNY via e-mail at Elsa.Araya@fdny. of any D-20 Certificate of Fitness holders who change employment locations so that their certificate of fitness cards can be updated. Notifications should include the certificate of fitness holder's name, certificate number, and their new employment location.

You will receive a courtesy notice of renewal 90 days before the expiration date. However, it is your responsibility to renew your Certificate. It is very important to renew your C of F before it expires. Renewals submitted 90 days (up to one year) after the expiration date will incur a $25 penalty in addition to the renewal fee. Certificates expired over one year past expiration date will not be renewed. New exams will be required.

The certificate can be renewed On-line, by Mail or in Person.

4.1 Renewal online If you are an individual, make sure you have your 12 digit Certificate of Fitness Access ID. This can be found on your Renewal Notice. If you do not have your Renewal Notice, your Access ID is your 8 digit Certificate of Fitness number and the last four digits of your social security number. If you are submitting renewals on behalf of a company's employees, the company must be approved by FDNY and have an 8 digit Company Code. To request approval, email pubrenew@fdny..
