IP-VISP/VOIP Manual Listing Input Instructions

All references to IP-VISP/VOIP is conveyed to mean those providers who have contracted with AT&T Advertising Solutions for the TIPToP or Standalone Directory Listings product offering.

Required: You must complete this field or your order will be rejected.

Conditional: If your order meets the conditional requirement (i.e. PLA on listed name), you must complete this field or your order will fail.

Optional: Information is helpful if supplied to match orders. Your order will not fail if an optional field is left blank.

Enter your 4-digit provider identifier code as provided to you by AT&T Advertising Solutions.

IP-VISP/VOIP PON # (Purchase Order Number) (Required)

Enter the IP-VISP/VOIP PON # associated with this order. The PON # is established by the IP-VISP/VOIP and is used to track IP-VISP/VOIP orders for rejects and reports. The PON # must be:

▪ unique (and may never be reused on subsequent orders);

▪ contain a minimum of 1 and a maximum of 16 characters;

▪ the last character of the IP-VISP/VOIP PON # must be numeric; and

▪ the only valid special character allowed is a hyphen (-)

▪ no other punctuation is permitted

Account Telephone Number (ATN) (Required)

Enter the 10-digit numeric account telephone number assigned to the end user.

Completion Date (Required)

Orders must be provided on the day service completes (the service order date), or on the effective date for a correcting order. This date is equal to the current date on all new, correcting and delete orders. The only exception to this is when an order is advanced—see AVL instructions below. The date entered should be in month-day-year format, i.e. 09-10-2004 for September 10, 2004.

IMPORTANT:

If you do not process your customer listing order when the service order completes due to a backlog, you must use the current date. (i.e. service order completes 9-10-2004 and today is 9-19-2004, you would use the current date of 9-19-2004 as the completion date of your listing order).

It is important to note that if you do not process your listing order when the service order completes, your customer will not have appearance in Directory Assistance (DA) until your order is completed.

Service Order Close Date

The service order close date is the final date that AT&T Advertising Solutions can assure listings will appear in all products.

Advance to Directory (Conditional--Needed only when AVL Requested)

Advanced Listing Code = AVL

(Not Applicable in West and Southwest Regions)

If the listing is not effective until a future date and is being supplied in advance of service order close for directory publication purposes, indicate the date on which the service order will complete in the Completion Date field and populate the Advance Listing Code field with the letters AVL. (The current date should not be supplied, but rather the date the service order is due to complete in the future). This future date must be prior to the actual printer ship date.

IMPORTANT:

Keep your original order in a follow up file for the future due date.

When an order is provided with a future completion date for inclusion in a closing directory, ANOTHER ORDER MUST BE PROVIDED WHEN THE ACTUAL SERVICE COMPLETES TO ESTABLISH THE LISTING IN DIRECTORY ASSISTANCE.

When the service completes, you must use the same order with the same order number as the original AVL order. This is necessary to establish the customer in directory assistance and ensure the listing in next year’s directory.

All components of the order should be the same, including the completion date. Orders are never held pending completion--they are processed as received.

Bus/Res Indicator (Required)

Enter B for Business or R for Residence.

New Connect Directory (Optional)

(Not Applicable in the East Region)

This field should be populated with a Y (yes) when a customer is establishing new service and is not switching from another carrier so that he will receive a local directory immediately. If customer does not want to receive a directory until the next issue is published, populate a N (no). If left blank, the default is N (no), since most customers changing carriers already have a directory and do not want another until the new issue is published.

In the Southwest Region directory order functionality is available on TC Directory Info Link.

Annual Delivery Quantity (Required)

(Not Applicable in the East Region)

Southwest Region

Requests for changes to the number of directories a customer wants to receive can be made via Service Orders.

Midwest and West Regions

This field should be populated to indicate the number of directories a customer wants to receive on an annual basis. Normal delivery is one directory in which the listing will appear will be delivered to the customer annually. See the policies on directory distribution and directory charging in the Pricing section.

Changes to desired quantities cannot be made through WP Listing Input Forms.

All other requests for changes in delivery quantity will be made through the Directory Order Center. See the contact list.

Yellow Page Heading Code (YPH) (Required on Business Accounts)

(Not Applicable in the Southwest Region)

The IP-VISP/VOIP must ask the customer to identify the nature of its business and decide on a heading code. This code is used to correctly place Business Listings in the AT&T Advertising Solutions(. The YP Heading Code list is provided by AT&T Advertising Solutions. (A list of current Yellow Page Heading Codes can be found in the References section of TC Directory Info Link). This field is required for all Main Business Listings in the Midwest and East Regions on New Connect orders. YP Heading Codes are not applicable to Additional Listings. (To request a Yellow Page heading for additional listing refer the customer to AT&T Yellow Pages( Sales). In the West Region, this field is required for all Main Business Listings and Additional Main Listings on New Connect orders.

Note: Future references to a customer’s YP Heading may not reflect the YP Heading Code on your new-in order. The customer may have modified the YP Heading, or the existing YP Heading was retained for a customer with previous YP advertising.

If an end-user wants to change their YPH code after it has been established in any AT&T Advertising Solutions state, the end-user must contact its AT&T Yellow Pages Account Exec or the Yellow Page Sales Offices as noted below:

YPH of 'NONE' / ‘DNC HEADING’

  Used only when customer does not want to be in the yellow pages.

'DNC Heading' is the equivalent of NONE in IL.

'SRL-Customer Requests No Appearance (NONE)' is the equivalent of NONE in CT.

The following is the YPH Code for 'NONE' /DNC Heading:

YPH of 'Secure Heading'    

Used when YPH is unknown and should be secured with the end user.

SECURE is the only YPH code allowed in MO, OK, KS, AR, TX. It is an optional entry. However, it not used on AT&T Advertising Solutions Southwest orders.

'Pending' is the equivalent of Secure Heading in IL.

'Unknown' is the equivalent of Secure Heading in CA, NV.

Correcting Order Indicator (Conditional)

Only place an X in this field if you are responding to a Query Notification form requesting a correcting order.

Correcting Order Per Query # (Conditional)

Only provide query number when submitting correcting order per Query Notification form received from AT&T Advertising Solutions. The correcting order query number must match the exact query number indicated on the Query Notification form. Each state has a letter indictor that is state specific. i.e.: W=Wisconsin.

Number of Caption Set-up Pages Attached (Conditional—if listings are to appear in captioned format)

Whenever listings are to appear in captioned format, a set-up letter must be attached. Indicate the number of caption set-up pages attached. See caption instructions on pages 20-23 for set up example.

Type of Account (Required on Government accounts)

(Not Applicable in the East & West Regions)

Circle the appropriate county, state, city, school or federal account identifier to ensure the listing is filed in the correct directory section.

IP-VISP/VOIP Contact Section (Required)

Use this section to identify the IP-VISP/VOIP's representative who submitted this order. Indicate the name, telephone number and fax number for this individual. This is the person who should be contacted if there are any questions regarding this order. This is also the person to whom a Query Notification form would be sent if necessary.

Main/Additional Listing and Delivery Information

Main listing information must already be established before submitting additional and/or foreign listings.

When sending a change order complete all the fields in both the Out Listing Information and In Listing Information sections.

When sending new orders populate the In Listing Information section of the form completely.

When sending delete orders only populate the Out Listing Information.

Delete orders to delete one of two duplicate accounts must be sent manually. Transmitting a mechanized order to delete a duplicate account will error out and require querying back to the IP-VISP/VOIP.

Record Type (Required)

Identifies the type of listing being submitted. This is a three-character field.

For Foreign listings the only valid Type of Listings are AL and CR.

For West Region the value of CR is only valid on Business Accounts.

List Type (Required)

This field indicates whether the listing requested should be listed, not listed in the white pages but available in DA, or not published at all.

Use the following codes to indicate the type of listing requested.

Note -When the List Type is 3, 4 or 5, The Record Type may only be LML.

Prior Level Status (PLS) (Required with caption listings)

This field indicates whether the caption is new or if one currently exists with that same header, sub-cap header and/or indent. Enter E if the caption is existing; N if it’s new.

Listed Telephone Number (Required)

Enter 10-digit telephone number, including area code. For example, enter the telephone number as 810-555-1234.

Alphabetic Listing Identifiers (ALI CODES) (Required)

The ALI Code is a unique identifier for an additional listing. An ALI code has a minimum of one character and a maximum of three characters. The first character must be alpha (alpha characters must be capitalized). Characters two and three can be alpha or numeric. The code should be provided in the ALI CODE box. IP-VISP/VOIPs will create their own code.

In the Southwest, Midwest and East, 11-99 and 1A-9Z are prohibited.

In the East the only valid entries are A-Z or AA-ZZ.

Omit TN Indicator (Required or Prohibited based on Cross Reference Application—only valid on cross-reference listings)

On a cross reference listing starting with the word “See”, this field is required, as the telephone number should not print. On a cross reference listing starting with “Call”, this field is prohibited, as the telephone number should print in this case.

Name (Required)

Enter the exact full spelling of the customer’s name. The listed name should be provided the way in which the customer wants it located in the directory. The field should be populated exactly as the name should appear in the directory. Populate using mixed characters, utilizing upper and lower case characters, for the correct appearance in the White Page directory. (See CLEC Online for specifications)

Last/Business Name:

The full spelling of the customer’s last name if residence or a business surname or the entire business firm name. There should be a space between each word of a business’s name.

First Name:

The first name and middle initial if desired of a residence or business surname customer. If a dual name listing is requested, enter the two names separated with space--ampersand—space.

i.e.: Mickey & Minnie

If the customer wishes to be found under both names in DA, complete the Dual Name Listing field as well.

Titles/Designations:

Captioned Listings:

When a listing is to be positioned as part of a caption, prior levels must

be identified on the form, along with their level of indent i.e.:

Proof Of Doing Business

Customers should provide proof of their listed name in the following circumstances:

• When a listing begins with a letter or a number, for example, “A Towing Co.,” “B Baby-sitting by Barb,” “1 24 Hour Locksmith.” These listings are generally intended to place a customer before other similar listings in the directory.

• Any name that, in the opinion of the service rep, is not customer’s valid business name.

Proof must be a form of County, State, US, or local government papers. For example:

• Business license

• DBA (Doing Business As) papers

• Federal or State ID

AT&T Advertising Solutions reserves the right to request a copy of this proof at any time. Other forms, such as business cards and stationery, are not acceptable.

BP/RP (Optional)

(Not Applicable in the West & East Region)

Southwest Region

BP (Business Professional) should be checked when the account has a business class of service and a surname listing. Listings that are business surnames are entitled to an appearance in the business and residence sections of a directory. Therefore, it is necessary to indicate the listing as BP.

If an account is a residence class of service with a humanitarian designation (MD, etc.) the listing is entitled to an appearance in the residence and business sections of a directory. Therefore, it is necessary to indicate the listing as RP.

Midwest Region

If the listing is a business surname different punctuation needs to be generated for correct processing. Therefore, it is necessary to indicate the account as BP. The program will then generate the appropriate punctuation.

Even though we do not request punctuation to be provided by the IP-VISP/VOIP, our system to produce the directory only recognizes where to place the customer listing (bus/res) section of the book by established punctuation standards. Surnames have long been recognized by the system as residential. This is why it is necessary to use BP to indicate if you want a business surname to appear in the Business section.

If an account is a residence class of service, i.e. Sororities and Shelters, select an RP account. This will drive the listing to the business section of business/residence split directories.

BP should be checked when the account has a business class of service and a person, or surname, listing. These listings are frequently accompanied with a designation, i.e. Mouse, Mickey Barbr. Checking this box will cause the listing to duplicate in both sections of a business/ residence split directory.

RP should be checked when the account has a residence class of service but has a firm name. RP listings are printed only in the Business section of a directory, since they are a firm name (i.e. St Adelberts Convent).

To avoid duplication of the listing, LNR is used to drive the RP listing strictly to the residence section, and LNB is used to drive the BP listing solely to business.

Dual Name Listing (Optional)

(Not Applicable in Indiana & the West & East Regions)

Some residential customers want their listing to appear with two first names, i.e. Mouse, Mickey & Minnie. Dual name is for Directory Assistance purposes only. In order for the listing to appear in DA under Mouse, Mickey and Mouse, Minnie, the DUAL NAME LISTING field must be completed. The box at the bottom of the name field should be checked to ensure this reversal in DA.

The last name must be the same in a dual name listing, i.e. Mouse, Mickey & Minnie. Mouse, Mickey & Duck, Daffy is not acceptable, as it does not reverse properly in Directory Assistance.

Service Address (Required)

A Service Address is always required. Addresses must be validated by using the Address Validation Inquiry that is provided on Verigate. Whenever the listed address is not a valid address, a valid Service Address must be provided in order to properly scope the listing in the proper directory. If there is no valid address on the order, it may not scope properly.

When changing a service address, the main and all additional listings must be shown in the In Listing Information section and again in the Out Listing Information section with the new service addresses.

Indicate the customer’s service address, including:

Prefix (Not Applicable in the West Region)

The address prefix is data that precedes the house number in the listed or service address.

House Number

Enter the house number in this field. A house number may be followed by a fraction, 1-4 alpha characters, or a fraction and a 1-character alpha character. A hyphen must separate the house number and any house number suffix, i.e. 111-1/2 Willow Lane; 2545-G W Main.

Directional (Dir’l)

Enter the street directional, i.e. 105 E. Main as N, S, E, W, NE, NW, SE or SW in the directional field.

Street Name

Enter the listed street name here in full spell format.

Thoroughfare

Enter the thoroughfare if required in the valid abbreviated format (i.e. Rd, Av, Cir) as referenced on the Address Validation Inquiry. The thoroughfare “Street” is usually assumed and should not be provided unless shown on the Address Validation Inquiry which is available on Verigate.

Suffix

The suffix to a street name identifies alphanumeric data that follows the street name. In some communities which are arranged in quadrants, the suffix directional is actually part of the street name, in which case it should be provided in the street name field. Where the directional is truly a suffix, enter the information in the suffix field.

Community Name (Required)

Enter the customer’s legal or postal community, spelled in full. Provide legal community name wherever possible for placement in the proper directory

Location

This is descriptive information about the main listing address, i.e. Apartment, Suite, and Floor. This information is used to expedite directory delivery and should only be supplied on a main listing. It will not print in the directory.

You may include up to 3 locations per main order.

Submit this information in abbreviated format as follows in the Location field. The location should be supplied as Flr 1; Suit 5, etc. Locations such as 1st Floor, 2nd Lot, etc. are unacceptable and will reject.

Zip Code (Required)

Enter the 5-character zip code associated with the listed address in the zip code field.

Omit Address from Directory (OAD) (Optional/Restricted)

In the Southwest, East and West, a customer may omit his complete address from the directory on any listing by checking the OAD box on the manual form. In the Midwest, a customer may omit his complete address from the directory when it is a caption indent by checking the OAD box on the manual form. Otherwise, a community name is required at minimum. When OAD is used, or when a community only is being provided as the listed address, be sure and provide a complete service address on the main listing.

Remote Call Forwarding/Listed Telephone Numbers that do not match the Service Address (Conditional)

A service that allows an end user, in one calling area, to receive calls made by callers in a different calling area without the calling party incurring a toll charge.

Tel No. Phrase

The “Tel No” (Telephone Number) phrase is always (except in the East) required on accounts with this type of service, and the listed address must represent the termination point of the service, with a community name required at minimum. In other words, if a customer’s service is forwarded through one central office to a remote location and the prefix is not local to the listed address exchange name, a Tel No phrase must be shown to indicate the exchange from which the service is being forwarded, i.e. Milwaukee Tel No. The exchange name should always be provided in full-spell format, although it may be abbreviated at the Publisher’s discretion. If the customer’s telephone number is ported from AT&T, a Tel No phrase would appear on the customer’s current CSR, which can be obtained from the AT&T Business Office, and should be included on the order to establish a IP-VISP listing with AT&T Listing Services. Without this information, the listing cannot be accepted by the database.

In the Southwest and Midwest orders for the listing to appear in the directory that covers the community in which the customer is located, a foreign/additional listing must be purchased. (For example, if the customer is located in Countryside, IL, and has an Oak Lawn Tel No., the main listing will appear in the Evergreen Park directory. If the customer wants to appear in the La Grange directory, a foreign/additional listing must be purchased for that directory.)

Exchange names are available on the Local Exchange Routing Guide. (The Local Exchange Routing Guide can be obtained through Telcordia Technologies’ Traffic Routing Administration (TRA) website at .)

Enter the Tel No. Phrase and community information in this field.

West Region

If Account Telephone Number is not local to Service Address because service is Remote Call Forwarding or Foreign Exchange, please check either RCF or FXS.

For each RCF or FXS line, subscribers receive one Main Listing in the exchange Directory (based on the account telephone number prefix).

A free listing is also provided in the White Pages (only) of the local directory, determined by the subscriber’s service address. (If two different directories are involved)

Exception: Nevada Subscribers only receive a listing in the exchange directory. A local directory listing must be purchased as an Additional Listing.

East Region

RCF lines permit Business end users in one calling area to receive calls made from callers in a difference calling area without the call party incurring a toll charge between the line and the subscriber’s location.

RCF is not an actual line on the customer’s premises. When calls are made to RCF number in a distant city, those calls are forwarded to the customer’s existing line(s) on their premises. Toll must exist between the RCF exchange and the listed address.

For each RCF line the Main Listing will be provided in the exchange Directory (based on the account telephone number prefix) and listed in Directory Assistance. RCF Main Listings include a free Yellow Page listing under the heading of choice. Additional listings may also be provided.

The listed address may be omitted, however, if an address is present the locality is required and spelled in full.

List Address (Conditional)

If the List Address is the same as the Service Address then this field does not need to be completed. Sometimes it may be appropriate for a customer’s listed address street name to be non-traditional, i.e. Oakland Mall, Prudential Building, etc. Additionally, some customers may not want their street address printed in the directory. Regardless of these two situations, all straight-line listings must have a valid legal or postal community name (caption indents may be entirely omitted via the use of the OAD indicator).

In place of the full physical address (Street Number, Street Name, Thoroughfare, and ZIP Code if necessary), the end-user may list:

1. PO Box 1234 – (PO Box not valid in the East Region)

2. Rt 3 Box 241-

3. Omit Address from Directory - (OAD on service order)

4. Street Name only (actual street name where the service terminates)

5. Building Name (actual building where the service terminates)

6. Mall or Shopping Center Name (where the service terminates)

Omit from Address Directory (Optional)

(Not Applicable in the Southwest & East Regions)

This field should be left blank unless the listing is to be omitted from the AT&T Advertising Solutions Address Directory. If the customer does not want to appear in the Address Directory, put an “X” in this field. (Customers who are non-list or non-pub are automatically excluded from this directory, as are additional listings, and the box need not be checked.) Direct mail firms use address directory. IP-VISP/VOIP’s should only check this box per customer request only to be removed from the list.

Directory Code and Subsection Names (Required on Foreign and Toll Free Listings)

An additional listing is considered foreign if the listing is to be placed outside the primary directory/section.

Foreign listings must be accompanied by the Directory Code (DIRIDL) in which the customer would like their listing to appear. In the Southwest Region, the subsection name must also be provided. A set of Directory Name Codes and Subsections are provided in the References section of TC Directory Info Link.

Toll Free #s (Not Applicable in the West Region)

Listings for Toll Free #s must be submitted as a foreign listing. See below for further information on foreign listings.

A listed telephone number can belong to only one provider. If the same service provider provides service for the main line and the toll free #, then that service provider can submit the listing service order to establish the additional listing for the toll free #.

 If a different service provider provides service for the toll free # than the main line, then the foreign additional listing for the toll free # should be established by that service provider.

Midwest Region

Toll Free phrase is never shown on the service order. If it is shown it will be removed in the system interface and will not print. To appear in the directory, “Toll Free” would have to be purchased by the customer as advertising. Request should be referred to AT&T Yellow Pages® Sales.

WPP: VIP and Custom Listings (Optional, Only Valid on Residential Class of Service)

(Not Applicable in the West & East Regions)

VIP (bold/script) and Custom listings are available to residential customers only.

If the VIP product (bold or script) is desired, check the appropriate box.

If a Custom listing is desired, check this box, and provide the text requested in

the Special Text field.

Delivery Address (Conditional)

(Not Applicable in the East Region)

Delivery Address should completed if the customer wants directories delivered to an address other than their Service Address. If the Delivery Address section is not completed directories will be sent to the Service Address.

Extra Listing Information (Conditional)

This field should be use to convey additional information regarding a listing. Include any other pertinent information in this section. Circle the appropriate selection as described below on the form. See the categories below:

Remarks/File As Information

This space should be used to indicate File As/PLA information or to indicate remarks such as a customer’s previous name in the event of a name change coincident with the transfer of service to an IP-VISP/VOIP.

This field should also be used when Letter Placement positioning is requested with a name that has multiple single letters with no spaces between, i.e. ABC Donuts. If the desire is that the finding word be printed together with no spaces between but filed in the directory as if there were spaces, this filing preference must be indicated in the File As field, i.e.

File as: A BC Donuts

This will cause the listing to file with the A’s, rather than with the ABC’s.

If multiple single letters have spaces between them, File As is not necessary, as the listing will file by the first letter, i.e. A B C Donuts will file with the A’s. See Alphabetizing Guide in the Rules for additional information related to File As.

LOI (Line of Information)

(Not Applicable in the Midwest & East Regions)

Southwest Region

This field is only applicable for residential customer only. Residence customers may add customized extra lines of information to a primary or additional listing in order to further describe the customer. A customer can purchase up to eight Lines of Information per residence listing and is limited to 34 characters per line, including spaces. Line of Information appears in italicized type directly below the associated listing and above the address. Line of Information can be purchased singly, in conjunction with a Signature or Personality Logo listing, or in a combination of all three features. Line of Information text is not passed to directory assistance.

West Region

Lines of Information (LOI) are only available with Caption listings and allow subscribers to include additional text with a Main or Additional Listing. These informational lines can be up to 36 characters in length and appear indented under the first line of the caption listing. Lines of Information cannot be used for advertising and cannot contain a telephone number other than the one associated with the Main or Additional Listing. It may be used to specify:

1. when, how, or where to call

2. occupation, hobbies, or interests

3. extension numbers (where “Ext” must appear just before the extension)

A Line of Information is required to use an address other than the service address and also required to use a US post office box outside the local service area in a Main or Additional Listing.

Example: A subscriber wants to list a totally different address than the Service Address. The Listed Address boxes would be left blank and the Service Address boxes would be filled in. The Listed Address would be shown in the LOI box at the bottom of the form.

Special Text

Text should be provided in this field when:

1. A cross-reference listing is requested. The referenced listing should be provided in this field, along with the word “See” or “Call,” i.e.

Call IBM (Not Applicable in the East)

A cross-reference listing must be submitted on a Foreign/Additional Listing Input Form.

2. A custom listing is being ordered. The text associated with the listing should be entered in this field. (Not Applicable in the West)

• A Telecommunications Devices for the Deaf (TDD) Listing is being ordered. End users can have listings with designations of TDD Only or TDD and Voice. When submitting a TDD Listing, please indicate this on your cover sheet submitting the order. (Not Applicable in the East)

Caption Processing Instructions

Caption Overview

Captions are a great way to organize listings within the white pages of the AT&T Advertising Solutions directory. A caption can contain several hundred listings or as few as two listings. Therefore, captions can be used in conjunction with either large or small businesses. Captions are established to avoid repetition. You can find multiple listings indented under a particular business name instead of having multiple straight line listings.

A caption arrangement may be used for grouping listings of a customer when two or more listings are involved for the same list name. A caption header is the name of the business. All associated listings are indented under the caption header. Indent levels always appear below the caption header and start 1 position to the right of the caption header starting point. Indents may appear at the same level, or indented 7 levels (5 in the East Region). Indents can be text, text with a telephone number, an address, or, an address with a telephone number. Caption headers in the West Region are only valid in the following manner:

A Caption Sub-Header is directive text that must be followed by an indent that is one level greater than the sub-header. Directive text sub-headers are used when the caption arrangement contains multiple indents that can be grouped within a sub-header category.

As with caption arrangements, indents in SLU sets can be from 1 through 6 levels. A SLU set can have 1 or, more indents. A SLU set must always appear sequentially in a data file, i.e. a SLU header followed immediately by one or more SLU indents

Orders for captions or indent sets should be submitted as follows:

Prior Level Status should be provided in the PLS field to indicate whether the caption is New (N) or Existing (E). Enter N or E.

Then degree of indent must be noted to indicate the position within the caption that the listing will appear. Degrees of indent are limited to five in the East Region, and seven in all other regions.

The name, including all headers, sub headers, prior levels, and the listing itself, should be provided in the Name field on the order.
