RESIDENT MANUAL

Welcome to Changing Faces & Agape Homes.

We, at Changing Faces & Agape Homes, understand how difficult it is moving to a new environment. We are here to assist you in reaching your goals, both personal and professional. We have designed this manual to help ease any confusion that you might be feeling and to ensure that you understand how the program operates.

Placement at Changing Faces & Agape Homes is very different than placement at a juvenile hall or a foster home. During Placement, you will receive ongoing counseling, participate in groups, learn to accept responsibility for your behavior and actions, increase your independent living skills, make changes necessary to live a socially responsible life, and to develop skills necessary for problem-solving and sound decision-making.

During the Intake Process you will be informed of Changing Faces & Agape Homes Rules and Regulations. All rules are enforced and guidelines are set to help you begin making positive changes in your life. Also, during the Intake Process you will learn about the different programs that Changing Faces & Agape Homes has to offer.

This manual will provide you with information about the Behavior Modification Level System. This system is designed so that you can measure your progress in the program. There are certain requirements that you’re expected to meet for each level. Both staff and your peers will give you feedback on how you are doing on each level. When you are working the program you will advance to new levels, allowing yourself to have more privilege at each level. Remember, all parts of your program are important, including school, work, activities, home passes, therapeutic services, doing your chores, how you interact with staff and other residents, et cetera.

There is a lot of information that you will need to learn about Changing Faces & Agape Homes, and much of it is explained in this manual. Refer to it to answer any question’s you may have. Remember, you can also talk to your peers or staff. We are all here to work together.

Remember, also, that this is YOUR manual. It is important that you refer to it when you are unsure of any area of the program.

GETTING STARTED THE RIGHT WAY

You’ve been interviewed and accepted into Changing Faces & Agape Homes. This means you’ve made commitments or promises to change, set goals for yourself, and you have agreed to follow the rules of Changing Faces & Agape Homes. Now it is time to start the program.

Changing Faces & Agape Homes provides you with the opportunity to make a fresh start and to improve certain areas in your life. From this point forward you will be taking steps to build a new reputation and life for yourself. This is based on how well you work the program, and how you interact with the people around you.

Here are a few suggestions that will make your stay at Changing Faces & Agape Homes a successful one:

1. Pay close attention and listen carefully

2. Cooperate

3. Ask questions when you are unsure of anything…there is no such thing as a “dumb” question

4. Develop a positive attitude

5. Remain open to new experiences

These are some of the ways that you can show people that you are working your program. Cooperate with staff and peers. Utilize the group to discuss your feelings and to solve problems. You need to be honest about your behavior and accept responsibility for your mistakes.

The staff at Changing Faces & Agape Homes hopes you successfully complete the Intake and move to the next part of your program. Remember we are here to help you. It is your program and you are the one responsible for making it a success.

Therapeutic Agents/Services

Group Counseling/Meetings:

Group meetings are held at least one time per week, for approximately one hour. Group meetings consist of all residents and a member of the treatment team. Participation in these meetings will assist you in developing problem-solving and relationship-building skills. Peer groups are also held weekly, and you are encouraged to attend these as well. These may be held with a member of the Management Team, Treatment Team and/or a Child Care Worker.

Groups can be the most important function of leading a successful program. During group, you will be involved in discussions about your program, childhood, future, etc. You will be able to get support and feedback from both your peers and staff.

B. Group Expectations

· Individual group members come to group prepared to talk about their personal agendas.

· A group agenda is an important issue or problem you are willing to talk about in a group.

· Agendas are clear and to the point. Group members have the responsibility to offer serious and honest feedback.

· The feedback needs to be supportive and non-confrontational.

The following expectations are necessary for group to remain safe and supportive:

Honesty: Only the truth is acceptable because dishonesty will hurt both you and the group.

Risk Taking Being honest and open takes courage and will build strength in you and trust within the group.

Confidentiality Feelings and information shared during the group must

stay in the group and are never to be discussed outside

the group.

Respect Respect must be shown for yourself and fellow members

of the group. Respect is shown by following the group

norms.

B. Group Norms

Groups are structured with specific rules called “norms.” Norms are rules or standards for behaving. Group members help their group by going over the norms of the group with each member.

A 2% grade increase will be given to each resident who successfully attends group.

Open non-defensive communication is very important in life and can be learned in group at Changing Faces & Agape Homes. Good relationship skills and problem solving skills require effective communication. The following list will help you learn the difference between defensive and supportive, open communication.

Individual Counseling

Every youth and non-minor dependent of Changing Faces and Agape Homes is

expected to participate in individual counseling. Changing Faces and Agape Homes has

counselors available to its youth for weekly sessions. Individual counseling is part of

every resident’s treatment plan in order to address individual needs and program

progression.

Family Counseling

Family counseling is available when appropriate. Not all of us have families that are

able to participate in counseling; therefore this will be determined by the

Treatment Team, Placing Agency and youth.

Equine

Changing Faces and Agape Homes are the proud owners of a Registered Paint Horse,

named Savannah as well as a rescued Thoroughbred, named Orlando. The youth of

Changing Faces and Agape Homes take turns going to the ranch to groom, ride, and

care for the animals. Horses are known to be very therapeutic for humans and our

youth agree. Involvement is not mandatory. It is voluntary.

ACTIVITIES PROGRAM

Changing Faces & Agape Homes offers residents the opportunity to participate in various activities, including sports and recreation. Residents will also be able to participate in group outings. Recreational activities include all forms of wellness, physical and spiritual programs. Residents may elect to attend the YMCA, the Abel Maldonado Community Center or local youth centers. Physical activities involve participating in everything from running and swimming to basketball and softball, just to name a few.

Residents are encouraged to participate in attending church services on Sundays. This is strictly each resident’s choice and Changing Faces & Agape Homes will not force anyone to participate.

Group outings occur on weekends and are based on how well you are working the program. Outings must be approved prior to leaving the home. Outings can be events happening in our community as well as surrounding communities. Outings range from attending sporting events, going to a beach or park, to going to Magic Mountain. It all depends on you and your willingness to participate.

Twelve-step group meetings in the community, such as Narcotics Anonymous and Alcoholics Anonymous (NA/AA) are offered. It is each resident’s personal choice whether or not to attend these meetings, unless they are a condition of Probation or an element of your Treatment Plan.

CHANGING FACES & AGAPE HOMES

EDUCATIONAL AND VOCATIONAL PROGRAMS

Each of you will be attending an educational institution of some kind, whether it be a local, public junior high or high school, perhaps a continued education or alternative education facility, or even a probation school.

In addition, each of you will be participating in training programs. The modules consist of cognitive development, emotional development, living skills, parenting, and various therapeutic agents. You may learn to develop a spiritual, physical and emotional maturity. Your training program will be clearly outlined in your Treatment Plan and specific to your needs.

For those of you who are eligible, you will be required to attend ILP classes that are offered in your area and local camps for ILP; such as, computer camps etc.

Independent Living Group

The Independent Living Group (ILG) is a program designed to provide life skills and transitional planning to the residents of the Changing Faces Inc. and Agape Homes. It is meant to help the residents plan for their future after the group home and give them essential skills needed for living independently. The ILG program is multiple workshops providing information on various subjects to the residents. The instruction is presented in different styles, from lecture to activities, quizzes and some field trips. The material is presented to the residents in a way that is interesting and fun. The ILG program encourages participation amongst the residents and offers additional one on one service if needed. By the end of the ILG program, each participant will have been educated on the topics of Career Interests and resumes; College Planning; Banking and Budgeting; Housing, Transportation and DMV information, Community Awareness; Personal Safety and Emergency Awareness/Preparation; Legal Knowledge and will have completed a Transitional Plan. The ILG program provides an overview of the workshop to the participants during the first class as well as rules and expectations. The participants are informed of the topics that will be presented and encouraged to ask questions or request additional information on any of the topics presented.

Transition Preparation

Residents may be referred to participate in transition services following successful completion of the Independent Living Group. Transition Preparation is designed to assist the youth to learn a variety of skills that build off of what was initially taught in ILG. If Transition Preparation is included in your Treatment Plan, you may participate in various goals and objectives stated in a Transition Plan devised by yourself and the Social Worker of Changing Faces and Agape Homes, be expected to gain employment, save money, work towards gaining a California Driver’s License, and other similar activities that will better prepare each you for your future..

Healthy Relationship, Parenting & Prevention Program

The Healthy Relationship, Parenting and Prevention (HRPP) program is designed to provide the education and skills necessary for awareness regarding relationships, as well as the effects of not taking proper precautions when making decisions. HRPP is a series of workshops introducing the reality to residents with regard to sex, relationships, STD’s and pregnancy. Residents are educated on various topics and involved in discussion about the different aspects to particular scenarios. The workshops explore the options available to individuals who may need the support as well as the long term effects of each option. Infant Simulation Program (ISP) is another part to the workshop, which allows the residents in the class to participate in caring for a baby for 3 days. The baby is downloaded with a program designed to offer a situation similar to caring for an actual living baby. The downloaded babies are programmed with different cries to communicate if they are hungry, need a diaper change, need to be rocked or need to be burped. The resident is responsible for the baby for the length of the download and required to care for the infant by responding to the baby by a monitoring system that matches the baby with the proper caretaker. The program is designed to track all mishandlings as well as make sure all the proper caretaking was given to the baby.

Substance Abuse Prevention

If you were placed here on a drug-related offense or have had problems in the past related to drugs, you will be enrolled in the Changing Faces & Agape Homes’ Drug Treatment Program, or an outside drug treatment program. Participation is part of your overall treatment at Changing Faces & Agape Homes and will be outlined in your Treatment Plan. Individuals who participate in drug treatment can be involved in an orientation, educational groups, individual counseling and attendance at AA and/or N.A. meetings as well as Relapse Prevention groups. The goal of these programs is to assist you in learning how to live a drug free lifestyle and increase safe alternative coping skills.

Life Skills and Behavior Modification Component

Changing Faces’ and Agape Homes’ daily program and level system is the life skills and behavior modification component to our comprehensive program. Residents receive privileges based on the effort they exhibit towards various aspects of their daily responsibilities, social interactions and activities, as well as program expectations. The goal of this component is to encourage our clients to achieve maximum benefit of reward from the program while enhancing life, coping, and social skills.

As residents progress through the levels, they are expected to exhibit an increasingly sophisticated range of skills based on ability, emotional maturity, and age. As their sophistication matures, their privileges and rewards increase and become additionally enticing. The social expectation at which is expected of the youth in our care, is based on social norms, prudent parent expectations, and the social-emotional level of functioning of the individual youth.

The caring adults employed by Changing Faces and Agape Homes are carefully trained to encourage the youth by providing a compassionate environment in which they can thrive. Through a structured and regimented setting, our employees guide the youth, teaching them to positively interact with their peers and maintain valuable relationships, build upon their strengths, follow rules and abide by all laws.

The life skills and behavior modification component of the program is accompanied by the therapeutic aspect. Many of our youth have endured complex traumas, come from less than desirable home environments, and/or struggle with various mental health issues. The life skills and behavior modification, as well as the multi-therapeutic component is integrated with various educational aspects in order to provide complete care to the youth so each individual can gain the skills necessary to enter the next phase of their adolescence.

The Life Skills and Behavior Modification Component of the program consists of a level system that has three primary levels, either Jr or Sr., one probability level, and a transition phase.

The Level System

Jr. and Sr. Level I Expectations:

This is the introductory phase of the program. At this level you will learn the rules of the program, become familiar with the staff members, and begin the counseling portion of the program.

* Learning and begin to utilize the problem-solving techniques discussed in groups

* Accept constructive feedback

* Set weekly short-term goals

* Help others

* Show respect for self and others

* Satisfactory school performance

* Comply with court requirements

* Willingness to participate in group counseling

* To remain in the program

* Set age appropriate long and short-term goals with the Treatment Staff

* Begin participation in a variety of therapeutic agents as specified by the Treatment Staff

Jr. and Sr. Level I Privileges:

* Earn up to $5.00 per week allowance

* Participate in Non-Paid activities with a passing grade

* May participate in up to two 6-hour day passes after two weeks of successful placement. Approval is dependent on re-unification status and the recommendation of the Treatment Team. Must have over 75%.

* May earn an extra phone call for positive behaviors to family and/or friends

Level 1 Fast Track:

On very few occasions, a client is placed at Changing Faces or Agape Homes after being transferred from another program due to reasons out of their control or as a step down to a lower level of care. In the event this occurs, a client may be considered for a Level 1 Fast Track. A resident must be recommended by his or her Social Worker, Assistant Manager of the program, and the resident’s Therapist. Behaviors while in their previous placement will be taken into consideration. Said client must maintain above an 80% in the program for 10 days, at which time may be considered to elevate to a level 2.

Jr. and Sr. Level II

After completion of Level I (30 days) you may then petition for Level II. This is decided by a Manager. Residents are expected to demonstrate a commitment to change their lives. Residents are also expected to increase their level of responsibility. They will develop values that reflect empathy and caring for themselves and others.

Sr. Level II Expectations:

* Set a positive example for peers by interacting in a caring and respectful way

* Utilize therapy, groups and other therapeutic agents to honestly look at behavior problems, emotional issues and personal values

* Set challenging weekly goals

* Accept responsibility for any wrongdoing without excuses

* Show good citizenship most of the time

* Learn to manage behavior, anger and other emotions

* Maintain consistent positive school performance

* Show an awareness of personal struggles

* Participate in and successfully complete ILG

* Participate in and successfully complete HRPP

* Participate in Transition Preparation, if necessary

* Participate in ILP, if necessary

Jr. Level II Expectations:

* Set a positive example for peers by interacting in a caring and respectful way

* Utilize therapy, groups and other therapeutic agents to honestly look at behavior problems, emotional issues and personal values

* Set challenging weekly goals

* Accept responsibility for any wrongdoing without excuses

* Show good citizenship most of the time

* Learn to manage behavior, anger and other emotions

* Maintain consistent positive school performance

* Explore your personal struggles and inner conflicts

* Explore and Participate in an outside activity such as; a youth group, sport, club, etc

Level II Privileges:

* Up to 3 weekend furloughs per month upon approval from Probation/Social Workers and Program

* Residents will be encouraged to obtain employment at age 16

* Allowance of $ 10.00 maximum per week

* Increased phone privileges and possible friend involvement

* Paid Outings, Movies, Boomers, etc.

* Eligibility of possessing a cell phone – 300 wards

* Increased community activities

Jr. Level III & Sr. Level III

We would hope at this level you will utilize what you have learned about yourselves and demonstrate a sincere investment in your daily lifestyle. Residents at this level will demonstrate support and leadership. Students will work towards graduation and plan for a transition into the community or adulthood.

Becoming a Jr/Sr. Level III:

* Upon petition by a resident for Level III status, staff and the Treatment Team will review the resident’s petition.

* Must be a Level II with a passing grade each week for a minimum of 45 days (6 weeks)

* Successful home/community furloughs, if applicable

* Positive peer and staff relations

* Minimal incident reports

* Positive school reports of 2.0 or above

* Satisfactory completion/ progress towards the goals stated in your Treatment Plan

* Active participation in counseling services recommended by the Treatment Team

* Any exceptions to the above criteria will be determined by the Management and Staff

Becoming a Sr. Level III continued:

* Satisfactory progress towards the goals stated in your Transition Plan

* Successful completion of Independent Living Group

* Successful completion of Healthy Relations/Parenting/Prevention Program

Sr. Level III Expectations

* Exercise interpersonal skills by showing respect, concern and care

* Develop realistic goals and plans for school, personal life, work, and transitioning

* Realistically look at your attitude toward the future

* Continue participation in all therapeutic agents as deemed appropriate by the Treatment Team

* Acceptance of family as it is and work towards creating a life that is independent from family, if applicable

* Participate in daily pro-social values and encourage values during groups

* Maintain involvement in the group- lead group(s) if applicable

* Complete a Relapse Prevention Plan, if applicable

* Participate in aftercare activities to include weekend home assignments, parental consequences, aftercare meeting assignments

* Continue to work towards the goals stated in your transition plan, maintain an active role in Transition Conferences, and seek assistance as needed.

* Maintain a 2.0 GPA or higher

* Exhibit leadership skills in the home and in the community

Jr. Level III Expectations:

* Participate in aftercare activities to include weekend home assignments, parental consequences, aftercare meeting assignments

* Maintain a 2.0 GPA or higher

* Maintain involvement in counseling and groups

* Explore age- appropriate activities

* Develop lasting friendships with your peers either in the home and/or community

* Explore your family dynamics and relationships, and work towards acceptance

* Exhibit leadership skills in the home and at school

* Participate in afterschool activities such as; youth groups, sports, clubs, etc

Level III Privileges:

All privileges enjoyed at Levels I and II, in addition to the following:

* Eligibility for 4 home passes per month (if recommended by the Treatment Team, has over 80%, and participating in re-unification)

* Possible extended furloughs upon approval from authorized representative

* Extra phone calls per week

* Allowance of up to $15.00 per week

* Eligible for later lights out on weekends or Holiday

* Eligible for room preference ( Larger room with bathroom)

* Extra laundry days

* Graduation participation

* Community Time - must have an 80% on weekly point sheet to be eligible

* ( Attending outings with friends, movies without immediate staff supervision) per Social Worker/ Probation approval

* Special Outings --Amusement Parks, Camping, Sporting events, etc.

* Eligibility of possessing a cell phone

Transition Level:

The Transition Phase in the program is for Youth preparing to enter their adulthoods within the next 6 months, unless otherwise decided by the Treatment and Executive Management Team. Said client has not engaged in behaviors resulting in a major Incident Report for a minimum of 60 days and has been able to maintain a Level III. At this stage of the program, Transition’s should be model residents, with maximum privileges; therefore, any refusal or rule violation at any time will be reduced back to level 3 for major incidents immediately. Changing Faces and Agape Homes reserves the right to incorporate consequences as deemed appropriate for the rule violation.

All residents who have entered the Transitions Stage have shown themselves to be leaders, have completed the goals stated in their Treatment Plan, and are preparing for their future. As leaders, you will be expected to uphold the policies and procedures of Changing Faces & Agape Homes. You will act as mentors to those who are on lower levels, instructing them on how to be successful in the program as well as outside of the program. Transition clients may incorporate their ideas on how to make the program function better overall. There are several benefits to becoming a Transition Level of this program, but higher expectations as well.

Transitions Privileges:

Matching Incentives – Changing Faces & Agape Homes will match savings accounts up to 1,000 of a resident’s earnings saved while in the program. The resident must show proof via bank deposits and or statements that he / she saved the money progressively.

Scholarships- Changing Faces currently offers residents the opportunity to apply for annual scholarships from both programs. Please submit your request in a formal submission of an essay of your accomplishments and future goals to changing2inc@ . They will then be forwarded to the board for review.

Allowance:

90%-100%=$ 20.00

80%-89%= $ 15.00

70%-79%= $ 10.00

Anything below a 70%= $1.00

Phone Time Allotment: Transitions may have up to 30 minutes daily for their phone time. Must have an 80% or higher. Below an 80% will result in level 2 phone privileges.

Activity Planning: Transition clients are encouraged to assist with weekly activities, including special events

Clothing Allowance: Transitions are given an additional $75.00 per year for their clothing allowance.

Meal Planning: Transitions may participate in meal planning monthly.

Chore/Bedroom Inspection: Transitions may be expected t assist staff in inspecting chores and bedrooms during the morning and evening shifts.

Paid Activities: Transitions with a 90% or above may have weekly paid outings

Transition Sanctions:

At this stage of the program, Transition’s should be model residents, with the maximum of privileges; therefore, any refusal or rule violation at any time will be harsh sanctions. Changing Faces and Agape Homes reserves the right to incorporate consequences as deemed fit for the rule violation. Transition Levels may receive a level demotion regardless of Restriction Status. Transition levels may receive up to 12 hours of community service.

Restriction Level:

Restriction Level gives you time to take an honest look at yourself, the mistakes you have made and the strengths and resources you have made to correct your actions.

You’re placed on Restriction Level when you engage in a major rule violation and/or have violated your probation.

Restriction level places you at Level one Allowance and Privileges, except non paid outings will not be permitted. The Restriction level will be evaluated weekly and may be continued pending positive behavior.

Positive behavior can result in an early release of restriction level.

Safety Level

Description:

Safety Level- A precautionary state of program that requires a no-harm/safety contract as a result of a youth exhibiting unsafe behaviors in which they have harmed, threatened to harm, or exhibited ideations of harm towards themselves or others. Examples of harm resulting in a Safety Level include; but are not limited to, cutting on self, suicidal ideations or gestures, threats of harm, running away.

Safety Restrictions:

All unsafe objects must be removed from youth’s possession/room. This includes; thumb tacks, nail clippers, glass, nails, shoe laces, power cords, ropes, and other things of this nature. (Shoelaces will be provided when leaving the home, and returned upon re-entry)

Youth may not participate in cooking, using knives, scissors, or other potentially harmful tasks.

Youth may only participate in the following activities; physical activity supervised by staff, transport with staff, and the youth’s services such as; school, D&A counseling, therapy, etc. All other activities and off site visits are restricted until Safety level is over.

Youth must keep their bedroom door all the way open when in their room.

Bathroom doors must be kept open approximately 2 inches when using the restroom and/or showering.

Youth will sleep on the couch for initial 48 hours following an incident of harm, as deemed necessary by a treatment staff member

Youth will participate in an additional counseling with appointment with a member of the treatment team.

Basic Skills

On the next few pages you will find a description of five basic skills and how to use them. These skills are very important to your Changing Faces & Agape Homes Program and will also help you in the future. If you do not understand what is written on these pages about the skills, ask one of your peers or a staff member to go over it with you.

Following Instruction

When you are given an instruction follow the guidelines below:

* Look at the person who is talking

* Show that you understand (“I understand”, OK or “ I will do it”)

* Do what is asked of you in the best way you can

* Let the person know you have finished

* If you are unclear about what is being asked of you, request clarification.

Accepting “No” Answers

* Look at the person

* Say “OK”

* Calmly ask for a reason, if you don’t understand

* If you disagree, bring it up later

* Take some time and plan what you’re going to say

Showing Respect

* Obey a request to stop negative behavior

* Refrain from teasing, threatening or making fun of others

* Allow others to have their privacy

* Obtain permission before using another person’s property

* Do not damage or vandalize public property

* Refrain from conning or persuading others into breaking rules

* Avoid acting obnoxiously in public

* Dress appropriately when in public

Accepting Criticism

* Look at the person

* Remain calm and quiet while the person is talking

* Show that you understand

* Try to correct the problem

* Most criticism is designed to help you

Disagreeing with Others

* Remain calm

* Look at the person

* Begin with a positive or neutral statement

* Explain why you have a disagreement with the decision

* Listen as the other person explains his or her side of the story

* Calmly accept whatever decision is made

* Thank the person for listening, regardless of the outcome

Anger Control Strategies

* If a person is talking to you, continue listening and acknowledging what he or she is saying

* Monitor your body’s feeling and how quickly you are breathing

* Breathe slowly and deeply

* Give yourself instructions to continue breathing deeply and relax your tense body areas

* If appropriate, calmly ask the other person for a few minutes by yourself

* While you are alone, continue to monitor your feeling and instruct yourself to relax

Coping with Anger and Aggression from Others

Rules and Regulations

HYGIENE

* Residents should be showering daily

* Tattooing and/or piercing is not allowed while in our program unless special permission is granted ie; ear piercing

* Visual facial piercings’ are not allowed. I.e. eyebrow, nose, lip .

Residents should practice good hygiene, such as wearing deodorant, brushing their teeth etc. Points will reflect if this is not done.

* FEMALE HOUSES- Feminine Napkins, Tampons etc. must be disposed of properly. These items must be wrapped in tissue and disposed inside the trash can. Only tampons may be flushed down the toilet. Applicators must also be wrapped in tissue and disposed in the trash can.

BEDROOMS

* Resident’s bedrooms must be kept clean. Drawers should be organized daily.

* Bedrooms will be inspected daily for neatness and graded accordingly

* Pictures and posters must be approved by staff and hung only on cork boards. Do not tack or tape items to the walls. No Gang affiliation or drug and alcohol glorification of any kind will be allowed to be displayed.

* Residents may have pictures, figurines, and personal items on their dressers. They must be kept neat, not cluttered.

* Bedding must be washed on your scheduled laundry day on a weekly basis. Beds must be equipped with a mattress cover, fitted sheet, flat sheet, pillow, pillow case, blanket, and comforter.

* Only the resident that resides in each room is permitted in that room.

* You will be held responsible for contraband found in your possession and/or belongings.

* We want you to feel comfortable in your environment so make your room your own… But keep it clean please…

KITCHEN AND FOOD

* Residents are not permitted to eat or drink outside of the kitchen or dining area, unless outside.

* Residents are expected to clean up after themselves. It is not the responsibility of the resident assigned to Dishes to clean the snack/lunch time dishes.

* Menus are to be followed in order to ensure healthy eating habits.

* Meals should be served family style.

* Snacks are scheduled by staff and should be set out for residents at appropriate times.

* Staff may restrict when and where a resident eats food that are brought in.

ACTIVITIES

* Residents will not be permitted to participate in outside weekend activities with a weekly percentage lower than %70. Paid activities will vary and will be offered at least 2x a month for levels 2 and above. Non paid activities will be offered weekly to residents that have maintained their privileges.

* Residents should be dressed appropriately for all outings. We do not allow slippers, pajamas, tops showing your belly, or short skirts/shorts to be worn out of the house. Staff will reserve the right to decide what is inappropriate as prudent parents.

* Residents should always use appropriate language and behavior, when out in public.

APPOINTMENTS

* Residents are expected to attend scheduled appointments. Refusals will result in signing a “Refusal to Participate” form and will be placed in the residents file. Consequences may apply.

RELIGIOUS SERVICES

* Church services are generally held on Sundays. Residents can elect to attend services.

Residents are encouraged to participate in youth groups and other church based activities once a level 2 is obtained unless otherwise specified.

MUSIC

* Stereo volumes must be kept to a minimal. It is unacceptable for staff to hear music in the office or common areas.

* Residents may not listen to CD’s/other music that has explicit sexual or violent language

LAUNDRY

Resident’s are expected to complete their laundry every week, according to the assigned laundry chart.

* Residents who do not complete their laundry on the day it is assigned will lose points in the Bedroom Inspection Category on the point sheets

* Laundry left in laundry area for more than two days will be bagged up.

HOMEWORK

* Residents are required to complete al least one hour of homework per day. Study hour points are designated for those who participate . Those who don’t will lose points.

Tutoring is encouraged for all clients; however is optional.

SCHOOL PARTICIPATION

* Residents are required to attend school daily. It is not an option. Refusals will not be tolerated.

* Residents may only miss school due to appointments, family emergencies or true illnesses. Excusable illnesses will be determined by staff and/or physician(s).

* Residents are encouraged to participate in extra-curricular school activities, ie: FFA, Sports, Cheer, dances,

DRUG TESTING

* Residents of Changing Faces & Agape Homes, Inc are required to Drug Test at any time, as deemed by Staff.

* Residents may be required to drug test for Probation and/or outside substance abuse programs as well.

* Residents refusing to test are subject to penalties if authorization was given to drug test.

*

AWOL

* AWOL is defined as leaving Changing Faces & Agape Homes Property or activity location without the permission of staff

* Resident who AWOL will be given consequences as stated in the resident’s sanctions. Continued AWOL’s may result in placement termination.

SHOPPING

* Residents will put in a written request to go shopping for clothes, supplies, etc when they are in need. These requests will be filled as deemed appropriate by Management.

* Shopping may not interfere with appointments, school etc.

* Chores and homework etc. must be completed before shopping can take place

EATING OUT

* Eating out is a privilege and will be monitored closely.

* Residents may eat out only after chores, homework and bedrooms are clean, if approved by staff

* Fast food may be a requested outing for weekends only, unless there is a special outing planned by Management during the week.

GROUPS:

* ILP is a necessary component to Changing Faces and Agape Homes’ program if you are enrolled.

* Independent Living Group is a mandatory component of your individual program is over the age of 15, unless otherwise specified.

* The Healthy Relationships, Parenting, and Prevention program is a mandatory component of your individual program is over the age of 15, unless otherwise specified.

* DMEI is a mandatory 21 week group counseling program that addresses topics specific, but not limited to; substance abuse, PTSD, relationship issues, and familial discord.

* Residents should be participating in groups. Groups are not voluntary.

* Occasionally, we have guest speakers from various agencies. Residents are expected to be polite, appropriate and courteous.

* Profanity is not permitted during any type of group.

* Residents will be required to participate in a Drug and Alcohol Program as deemed necessary by Management.

* N.A. & A.A. will be available on a weekly basis, and may be required as stated in your treatment plan.

THERAPY

All residents of Changing Faces and Agape Homes are expected to participate in individual and group therapy.

Each resident may be expected to participate in more than one type of therapeutic agent.

Therapeutic Services will be referred by the Treatment Team and stated on your Treatment Plan

EMPLOYMENT

* Residents who are of legal age to work and obtain a work permit, are encouraged to do so.

* Residents will be required to sign a work contract once a job is obtained.

* It is staff’s responsibility to ensure that resident’s arrive at work on time and picked up promptly.

* Once a job is obtained, Changing Faces & Agape Homes will help residents establish a secure savings account to prepare for transitioning. Changing Faces & Agape Homes will also recommend a % of each check be deposited into their personal savings account.

PERSONAL BELONGINGS

* We do not encourage Residents borrowing each others belongings. This includes; but is not limited to, clothing, CD’s, Ipod’s, shoes, etc. If a resident chooses to loan personal items to other residents, Changing Faces & Agape Homes will not be responsible for damaged or missing items.

* Residents are not permitted to be in each other’s rooms.

* Changing Faces & Agape Homes guarantees items and/or money that is entrusted to Changing Faces & Agape Homes Management, only.

* Belongings and/or money kept in the residents possession is their own responsibility.

* Residents are not permitted to have more than $20.00 on their person at any one time.

VEHICLES:

* Residents are required to wear safety belts at all times.

* Residents should not yell or use hand signs out of the windows.

* Residents may not touch the steering column of the vehicle or the driver’s seat.

* There will be no eating or drinking while in the vehicle.

* The volume of the radio is according to staff’s comfort level.

PHONE POLICY

* Resident may not answer the phone, unless instructed to do so by staff . Answering ( Changing Faces & Agape Homes Resident Speaking)

* Residents are permitted to use the phone to call their authorized representative, social workers, CASA workers, attorneys, Licensing or probation officers at any time; however, may be required to wait up to 15 minutes in order for staff to complete a task, or make the phone available.

* Residents may not use the phone after 9:00 P.M. unless an exception has been granted by Management.

* Residents may have make-up calls only if the person they are trying to reach is not available or if they miss their phone time due to counseling, appointments or work.

* Staff must DOCUMENT the phone call and dial for residents who wish to make calls.

* Residents may not contact individuals in which contact is not permitted. Changing Faces and Agape Homes reserves the right to deem whether or not a relationship with a peer/friend is inappropriate

* Staff reserve the right to end any phone call that contains inappropriate content.

All levels- Parents that have re-unification plans may call their child up to one time per day, for no more than 10 minutes.

Level 1:

Phone days are ,Mondays and Fridays to contact their approved family members* for a maximum time of 30 minutes.

Those with an 80% or above may have an extra 15 minutes with friends and/or family on Wednesdays.

Level 2:

Phone days are Tuesdays, Thursdays, and Sundays to contact friends and/or family for a maximum time of 30 minutes.

Level 3:

Phone days are every day for a maximum of 30 minutes. If they have 80% or above. Below an 80% may use the phone on Monday, Wednesdays, and Saturdays not to exceed 30 minutes. Level 3's may contact family and/or friends.

Restriction:

Restriction level may have a maximum of 30 minutes with family members on Tuesdays and Sundays.

All levels may contact their Social Workers, CASA's, Attorney's and Community Care Licensing as they wish.

Visitation/ Home Pass Policies

On Site Visits:

All levels are permitted to have on site visits with approved family members, immediately upon placement. The family members will be decided by the client’s authorized representative. Each resident’s visiting schedule will depend on the authorized representative.

All levels are permitted to have on-site visits with their CASA’s, Big Brother or Big Sister, or other appropriate agency contact immediately upon placement, as deemed appropriate by the authorized representative.

On site friend visits:

Level 1, 2, and 3:

Residents that have been placed at Changing Faces for a minimum of 10 days and that have displayed appropriate behaviors, may be allowed a friend to visit, with the following rules in place:

Said client must have at least an 80% and zero consequences pending.

Each visit will be scheduled with Management and pre-approved.

The location of the visit will also be deemed by Management.

At no time will a friend on current probation be permitted to visit one of Changing Faces or Agape Homes’ clients, unless it is an immediate family member.

Changing Faces and Agape Homes Management reserves the right to restrict, deny, or terminate any person from visiting its program, immediately. Changing Faces and Agape Homes are considered prudent parents and will exercise their judgment regarding peers and their potential influences.

*Changing Faces and Agape Management reserves the right to change or alter the visitation rules at any time*

Off Site Home Pass policy:

All levels- As long as a client has been placed at CF or Agape for more than 2 weeks, a passing grade, and no consequences on the board, then they may have off site visits with their CASA workers, Big Brother/Sister.

Level 1 (after 2 weeks of placement) MAY be permitted up to 2 day passes of a maximum of 6 hours off site with the individual(s) stated in the Family Reunification Plan, provided by the authorized representative. The off-site pass will be permitted if deemed therapeutic to the resident and approved by the placing agency.

Level 2’s MAY be permitted up to 3 weekends per month off site with the individual(s) stated in the Family Reunification Plan, provided by the authorized representative. The off-site pass will be permitted if deemed therapeutic and/or appropriate for the resident and approved by the placing agency. If overnight’s are not permitted by the placing agency, then level 2’s are applicable for up to 6 day passes per month. Please note* all residents must have at least 2 day passes prior to overnight visits.

Level 3’s MAY be permitted to every weekend per month off site with the individual(s) stated in the Family Reunification Plan, provided by the authorized representative. The off-site pass will be permitted if deemed therapeutic to the resident and approved by the placing agency. If overnight’s are not permitted by the placing agency, then level 3’s are applicable for up to 8 day passes per month.

Restriction Level MAY NOT participate in off site visits.

Please note* all family visits must be pre-approved by the placing agencies. Changing Faces and Agape Homes does not possess the right to permit clients to have visits with family members due to court orders and restrictions. All visits are subject to flexibility based on the placing agencies needs and recommendations as well as the treatment needs of the youth. Overnight and/or day visits with extended family or like individuals, not specified in the reunification plan may also be approved for visits. Level 3’s will not be permitted more than 3 weekends per month.

RESIDENT SANCTIONS:

Refusal to do chores= 0's and 7's in the following areas:

* Initiative

* Weekly Chores

* Following Program Rules

* Positive Role Model

* Follow Directives

Level 2 Same consequences

Level 3 or Graduate will receive zeroes in the above areas

Profanity Toward residents or staff

Incident reports are to be written for all Level’s.

Level 1 Zeroed out Shift

Level 2 Zeroed out Shift and up to 4 hours of community service

Level 3 Zeroed out Shift and put on Restriction for 1 week

Transitions Zeroed out Shift, 4 Hours of Community Service and Restriction for 1 week.

Refusal to attend school or Suspension (not due to illness, appointment or court)

All levels will receive an Incident Report that will be sent to the appropriate agency, a zeroed shift, and possibly receive two or more days of restriction. Depending on the number of refusals, levels etc.

Cutting Classes

When the school calls to report a class missed. The resident will be have (1) day to clear the absence and provide the clearance by way of attendance office and bring proof to the home. If this is not done by the following day then the following will occur:

Levels 1 and 2 will receive a zeroed out shift and a loss of weekend privilege.

Levels 3 and Graduates will receive a zeroed out shift, loss of weekend privileges and 2 or more days at Restriction Level(depending on the number of previous cuts)

Detention

First Offense (all levels) a zeroed out shift

Future Offenses may result in a zeroed out shift, and loss of weekend privileges.

Residents entering other resident(s) bedroom

Level 1's first time- no sanction with a verbal warning

After the 1st time any other times will result in zeroed out shift and an in-house incident report

Level 2's and 3' = zeroed out shift and an in house incident report

Residents eating or drinking outside of the kitchen and dining room

(to keep your floors clean and house nice )

Level 1's first time- warning

After initial warning= Zeroes in the following areas:

* Following Rules

* Positive Role Model

* Shows Initiative

* Follows Directives

Levels 2's and 3's will automatically receive the above sanctions and one hour of community service

Behavioral Issues while on activities or out in the community:

Zero’s in the following areas:

* Following Program Rules

* Positive Role Model

* Shows Initiative

* Follows directives

* Follows Treatment Plan

* Respect for Staff

* Respect for Peers

* Positive Attitude and Communication

Level 2's, 3's or graduates will receive a zeroed out shift and receive 1 or more days on restriction depending on severity. Outing will also end immediately for entire group.

Theft

All Levels will receive an Incident Report, zeroed out shift, store management will be alerted and extension of Level 1 status or restriction and may result in an automatic level demotion for any one who possess’ a level 3 .

Fighting

All levels will receive an Incident Report, Zeroed out shift, staff will notify local law enforcement (depending on severity), and two weeks extended level one or two weeks restriction. Other sanctions may apply. Management reserves the right to decide what is appropriate. If you are struck by another resident and do not hit back it is your right to press charges against that resident.

Rough Housing

Due to the possibility of injuries occurring we do not promote rough housing.

Resident(s) who rough house may be zeroed out. Further consequences may apply depending on severity.

Smoking

All Level’s will receive a zeroed out shift and incident report will be written and sent to the appropriate agency representative.

2nd offenses will result in a zeroed out shift, community service hours and/or restriction. Continued use of tobacco may result in essays, educational projects, or other consequences to promote a healthy lifestyle as deemed appropriate by the agency.

Music

Staff reserves the right to determine when music lyrics are too raunchy and when the volume is too loud.

Residents who refuse to turn the volume down on their stereos or change the music may :

Level First Offense: Have the stereo confiscated for one full day

After the first offense: Staff may confiscate the stereo for two or more days.

Residents who refuse to get off the phone when their phone time is over.

Level 1's may receive zeroes in the following areas:

* Positive Role Model

* Follow Directives

* Following Rules

* Shows Initiative

* Respect for staff

* Positive attitude/Communication

Level 2's and 3's will receive the above sanctions as well as a decrease in their next allotted phone time.

Unauthorized Phone Calls

Incident reports are to be written for all Level’s.

Level 1 Zeroed out Shift

Level 2 Zeroed out Shift and loss of weekend outing

Level 3 Zeroed out Shift and put on Probability for 1 week

Refusal to Drug Test

If residents are asked to drug test and refuse, they will then be assumed as a dirty UA and consequences for such will apply.

Dirty Urinalysis

Level 1 will have two weeks added to their level one status and be zeroed out for that said shift

Levels 2 and 3- Incident Report, Zeroed out shift, and two weeks of Restriction

AWOL

Level 1- two weeks extension of Level one

Level 2 -two weeks at Restriction Level, possible demotion if several awol’s have occurred.

Level 3 - 3 Weeks restriction -

All levels will receive an Incident Report and a zeroed out shift

Employment

Residents who are employed are required to remain at their place of employment unless arrangements have been made with management. Residents who fail to remain at their place of employment are considered AWOL and the same sanctions will be given.

Personal Belonging

Theft of another residents personal belonging will result in the following: zeroed out shift

To include it may also include being placed on restriction level, depending on the severity.

It is never okay to steal from one another. We do not take lightly on dishonesty and you will be held accountable.

Destruction of personal belongings: Zeroed out shift and deduction in allowance to cover the cost of the belonging, restriction for one or more days (depending on severity) and possible community service.

Sexual Behaviors

Residents that obtain hickeys and/or engage in sexual activities with other residents will receive an Incident Report sent to their authorized representative, automatic zeroed out shift as well as a loss of weekend privileges. Additional contracts, services, educational classes, and/or consequences may apply.

Vehicles

Failure to wear seatbelt:

All levels will receive an automatic zeroed out shift and 1 hours of Community Service for each incident. .

Yelling to vehicles, people etc. while inside the van will result in the following:

All levels will receive an automatic zeroed out shift and 1or more Community Service Hours.

Attempt to Destruct or Destruction of Property within or outside the vehicle will result in:

1 to 2 weeks of probability, depending on severity and level.

Depending on Severity, the resident may receive $1.00 per week in allowance money until the cost for the damage to the vehicle has been covered or possible termination of placement.

Eating while inside the vehicle:

All levels will receive a zeroed out shift and will have to clean the van inside and out upon returning to the house rain or shine. Or will be given 2 hours of community service and Horse Stables for the following Saturday.

Theft or Destruction of Staff’s Personal Belongings

May result in automatic termination from placement or other consequences deemed appropriate

Climbing on the Roof(Princeton)

All levels will receive an incident report, a zeroed out shift and 2 or more days at Restriction Level. Repeat offenders will automatically be placed on one-week of restriction and will receive the same sanctions listed above.

Entering the Staff Office without permission

Level 1's will receive a warning when first placed, then receive a zeroed out shift if continued, resident will receive Community Service on Saturdays in Graffiti Clean Up or Horse Stables.

Level 2's and 3's will receive an automatic zeroed out shift, depending on the number of incidents may receive loss of weekend privileges. .

Refusal to participate in Therapeutic Agents (D&A, Individual, group, NA/AA, TBS, Etc):

Refusals will result in an incident report to their authorized representative, a zeroed shift and depending on the number of incidents may result in Community Service Hours and/or a loss of privileges ie; community time.(The same sanctions will apply to those who display inappropriate behaviors while in therapy)

The following offenses will result in, an Incident Report, a zeroed out shift and one to two weeks at Restriction Level:

Fighting -

Dirty Drug Test

AWOL

Threatening Behavior (Staff/peers)

Paraphernalia

Piercing a body part (If more than one resident is involved then both will receive consequences depending on level)

Tattoos (If more than one resident is involved then both will receive consequences depending on level)

Property Destruction

Graffiti (inside the home, vehicles, etc.)

Gang- Related Behaviors ie; throwing up gang signs

Consuming Alcoholic Beverages

Contraband(lighters, matches, sharps, etc.)

Automatic Level drop for level 3's may occur for

Physical Aggression

Theft in store

Property/Vehicle Destruction

Threatening Behaviors

AWOL

Drug/Alcohol Use

Incident Reports

It is your right as a resident to receive a copy of any and all incident reports that are filed against you. All zeroed out shifts (except entering into another resident’s room) should be followed with an Incident Report whether it is an in house incident or an incident that will be forwarded to your representative. If a resident receives a zeroed out shift without an incident report, that shift will be given back to the resident by management.

Grievances

You may file a grievance any time you disagree with any element of your program, such as a sanction or consequence you receive for breaking a rule, or perhaps you disagree with a decision made by a staff member, or maybe you disagree with a house rule. You have a right to address your concerns by filing a grievance form. The grievance forms are available to you for your use and are located in the Resident Forms filing box. Fill out your form thoroughly, be specific about your concerns and give as much detail as possible. When you have completed your form you may turn it in to a manager. If your grievance involves a manager, you may turn your form in directly to the Executive Director or Human Resource Director office by faxing to 805-937-5473. Your grievance will be reviewed and hopefully resolved within 72 hours.
