Convenient blood work options

With the Otsuka Laboratory Support Program, you can get your Risk Evaluation and Mitigation Strategy (REMS) required blood work done for free, if your insurance does not cover it.

Free lab work at LabCorp or Quest Diagnostics

Before you go to the lab, remember to ? O btain an Otsuka Laboratory Support Program Requisition

Form from your doctor. Make sure the requisition form has the Otsuka logo on it, to avoid billing issues ? B ring the requisition form with you to any Quest Diagnostics or LabCorp location to get your REMS-required blood work done for free ? O nly REMS-required lab work (ALT/AST/bilirubin/ALP) is covered

Free home collection of blood sample

Make an appointment for a phlebotomist to come to your home to collect a blood sample for your REMS-required blood work. You must schedule your appointment 2-4 weeks in advance, to allow us to find a phlebotomist in your area.
