Adverse Drug Reaction:

Would you recognize one if you saw it…?

Has your patient had an UNEXPECTED effect from a medication?

Is your patient having SIDE EFFECTS from one of their medications?

Is your patient having an UNDESIRED EFFECT from drug therapy?

Are you giving your patient one medication because of side effects from another medication?

Have you had to administer a PRN antidote (i.e. Benztropine, Naloxone, Diphenhydramine)?

Has your patient developed a new DRUG ALLERGY?

The need for a reversal agent or antidote may indicate that an adverse drug reaction has occurred:

Please fill out an ADR report in Midas!

If you answered YES to any of the above questions, then your patient may be having an ADVERSE DRUG REACTION & it needs to be reported!
